血浆纤维蛋白原水平与突发性聋发病及预后关系探讨

【摘要】 目的探讨血浆纤维蛋白原水平与突发性聋的发病及预后间的关系。方法计算机检索Pubmed、Embase、Cochrane Library、中国生物医学文献数据库(CBM)、万方、维普、中国知网(CNKI),并手工检索纳入文献的参考文献。检索年限从建库到2020年1月,内容涉及突发性耳聋及血浆纤维蛋白水平。按照纳入与排除标准由两位研究者独立筛选和提取资料,采用AHRQ清单评价纳入文献质量,采用Stata MP14进行Meta分析。结果共纳入18篇文献,14篇文献分析发现突发性聋患者血浆纤维蛋白原水平高于正常听力者,且差异有统计学意义(Z=11.86,P<0.01);6篇文献按照听力曲线分型行描述性分析发现全频下降型突发性聋患者血浆纤维蛋白原水平最高;4篇文献行预后分析发现有效组及无效组的突发性聋患者治疗前血浆纤维蛋白原水平无统计学差异。结论血浆纤维蛋白原水平的升高预示突发性聋发生的风险增加,且在全频下降型突发性聋的发生发展中可能扮演着更加重要的角色,而治疗前血浆纤维蛋白原水平与突发性聋患者的预后无关。更多还原

【Abstract】 Objective To investigate the relationship between serum fibrinogen and the pathogenesis and prognosis of sudden sensorineural hearing loss(SSNHL).Methods The Pubmed,Embase,Cochrane library,CBM,WanFang Data,VIP,CNKI were searched for articles involving serum fibrinogen and SSNHL before January 2020,and manual searching for the references of included studies.Literatures were analyzed by two investigators according to the standard of inclusion and exclusion independently and their quality was evaluated using the AHRQ criteria.Meta-analysis was performed by Stata MP14 software.results Eighteen articles were selected.The analysis of 14 literatures found that the serum fibrinogen level of SSNHL patients was higher than that of control group,and the difference was statistically significant(Z=11.86,P<0.01).Descriptive analysis of 6 literatures according to the classification of hearing curve found that the plasma fibrinogen level was highest in full-frequency descending SSNHL patients.Prognostic analysis of 4 literatures found no statistically difference between the plasma fibrinogen level before treatment in effective and ineffective SSNHL patients.Conclusion High serum fibrinogen is a risk factor for sudden sensorineural hearing loss and it may play a more important role in the occurrence and development of full frequency descending SSNHL.However,there is no significant correlation between SSNHL patients’ prognosis and plasma fibrinogen level before treatment.更多还原
