Argon laser with intravitreal injections of conbercept therapy for macular PCV

【摘要】 Purpose:To evaluate the efficacy of combination therapy of Argon laser photocoagulation with intravitreal injections of conbercept in Polypoidal choroidal vasculopathy involving macula.Methods:A case was reported,whose main complaint was diminution of vision in the left eye for one month.Speciality examination:Best corrected visual acuity(BCVA) in the left eye was 0.1;from the fundus photo and optical coherence tomography,we could see the presence of orange-red subretinal nodules,around which a large PED and massively submacular hemorrhage.Treatment was performed with an argon green laser 532 mm with a focal contact lens using the following parameters:100 microns to 200 microns spot size and 100-150 ms duration and enough power to achieve a greyish-white burn to the active polypoidal lesions.Laser could be repeated at more than 1 session if closure was believed to be incomplete at follow-up.Meanwhile,the patient received monthly intravitreal injections of conbercept for 3 months.This patient was followed-up over 36 months with best-corrected visual acuity(BCVA),OCT(including central retinal thickness(CRT),subretinal fluid(SRF) thickness,pigmented epithelial detachment(PED),hemorrhage) and FFA.Results:Five months later,the therapy helped in decreasing the mean central retinal thickness and resulted in stabilization of the disease with visual improvement from 0.1 to 0.7 with PCV involving macula.ICGA showed no evidence of any active polyps.The patient was followed up for almost three years,FFA revealed staining in the macula corresponding to RPE fibrous scar and BCVA improved significantly to 1.0 and was maintained for more than one year.Conclusions:The guideline providers a standard treatment protocol in management of PCV patients,but it may needs to be individualized.Combined Argon laser with intravitreal injections of conbercept therapy is a way to treat of selective cases of macular PCV.更多还原
