超声处理对牙本质玷污层性状及粘接强度的影响
The effect of ultrasound on the properties and bonding strength of dentin smear layer

【摘要】 目的 :探究超声处理对牙本质表面玷污层性状及对通用型树脂粘接剂自酸蚀模式下粘接强度的影响,以期为临床应用提供一定的理论基础。方法 :将40颗人无龋下颌第三磨牙随机分为两组,一组用320目碳化硅砂纸打磨,另一组用320目碳化硅砂纸打磨+C尖超声处理,运用扫描电子显微镜观察牙本质表面玷污层的性状;处理后的离体牙分别用Rhodamine B染色的2种通用型树脂粘接剂(Clearfil Universal Bond和All-Bond Universal)粘接复合树脂,在激光扫描共聚焦显微镜下观察粘接界面的渗透情况;最后制作长方体微拉伸试件进行微拉伸强度测试。结果 :扫描电子显微镜显示经过320目碳化硅砂纸打磨后,牙本质小管几乎完全被堵塞,未见明显牙本质小管暴露,而经过320目碳化硅砂纸打磨+C尖超声处理之后,可见大量牙本质小管暴露;激光扫描共聚焦显微镜显示经过C尖超声处理过后粘接剂更能够沿牙本质小管渗透入牙本质深部;微拉伸强度测试结果表明超声处理能够增强牙本质自酸蚀模式下的粘接强度。结论 :超声处理能够部分去除牙本质表面玷污层,暴露牙本质小管,增加通用型树脂粘接剂的渗透深度以及粘接强度,是一种具有临床转化潜能的方法。
【Abstract】 Objective: To explore the effects of ultrasound on the surface of the dentin smear layer and the bond strength of the universal resin adhesive under self-etching mode to provide a certain theoretical basis for clinical application. Methods: 40 mandibular third molars without caries were randomly divided into two groups, one group was polished with 320 mesh silicon carbide sandpaper; the other group was polished with 320 mesh silicon carbide sandpaper followed by C-tips ultrasonic treatment. The scanning electrons microscope(SEM) was used to observe the surface of the dentin. The treated teeth are bonded with two universal resin adhesives(Clearfil Universal Bond and All-Bond Universal), respectively, and the penetration of the bonding interface was observed with a confocal laser scanning microscope(CLSM) after Rhodamine B stained. Finally, the micro tensile bond strength test was conducted to test the adhesion. Results: The SEM showed that after polishing with 320 mesh silicon carbide sandpaper, the smear layer of the dentin surface was scratched, and the dentin tubules were almost completely blocked, with no obvious dentin tubules exposed. After ultrasonic treatment with C tips, the scratches were reduced, and a large number of dentin tubules were exposed. CLSM showed that the adhesive can penetrate the dentin along the dentin tubules more deeply after the C-tips ultrasound treatment. Micro tensile bond strength tests showed that ultrasonic treatment can enhance the bonding strength, and there was no significant difference between the two universal resin adhesives. Conclusions: Ultrasound can partially remove the smear layer on the surface of dentin, expose the dentin tubules, and increase the penetration depth and bonding strength of universal resin adhesives under the self-etching mode, indicating that the ultrasonic wave can achieve clinical transformation potential.
