【摘要】 目的:患者,女,48岁,半年前于外院修复失败后。上下颌多颗牙缺失,咬合关系完全丧失,面容苍老,口角塌陷,发音不清。来我院要求重新修复。方法:口腔检查:11、12、16、17、21、22、25、26、27、31、32、36、37、41、46、47缺失,13、14、15、23、24、33、34、35、38、42、43、44、45残冠,15、35、38、42叩诊(+)、牙齿I°松动,全口牙龈红肿,质地松软,BOP(+)。X线:13、15、35、38、42根管充填欠完整。咬合关系丧失,面下1/3缩短,口角塌陷,愁苦面容,精神状态欠佳,发音不清。诊断:1.慢性牙周炎。2.13、14、15、23、24、33、34、35、38、42、43、44、45牙体缺损。3.上颌Kennedy第一类第一亚类牙体缺损,下颌Kennedy第二类第二亚类牙体缺损。治疗计划:牙周基础治疗;13、15、35、38、42根管再治疗;13、14、23、24、33、34、43、44、45套筒冠+15、42覆盖义齿混合支持式咬合重建。结果:修复体戴入1个月,患者咬合功能恢复,发音清晰,面容改善,精神状况良好,患者表示非常满意。结论:套筒冠与覆盖义齿的联合应用可为可摘局部义齿提供良好的固位和稳定作用,同时可有效保护基牙。更多还原

【Abstract】 Objective: The patient, female, 48 years old, failed to repair in the external hospital six months ago. The occlusal relationship of multiple teeth in the upper and lower jaws is completely lost, the face is old, the mouth is collapsed, and the pronunciation is unclear. Come to our hospital and ask for a new repair.Methods: Oral examination: 11, 12, 16, 17, 21, 22, 25, 26, 27, 31, 32, 36, 37, 41, 46, 47 missing, 13, 14, 15, 23, 24, 33, 34, 35, 38, 42, 43, 44, 45 residual crown, 15, 35, 38, 42 percussion(+), teeth I ° loose, full mouth gum dark red, probing bleeding. X-ray: 13, 15, 35, 38, 42 tube filling is not complete. The occlusion relationship is lost, the lower third of the face is shortened, the mouth is collapsed, the face is bitter, the mental state is poor, and the pronunciation is unclear.Diagnosis: 1. Chronic periodontitis. 2.13, 14, 15, 23, 24, 33, 34, 35, 38, 42, 43, 44, 45 tooth defects. 3. Upper jaw Kennedy first class first subclass tooth defect, mandibular Kennedy second class second subclass tooth defect.Treatment plan: periodontal treatment; 13, 15, 35, 38, 42 tube retreatment; 13, 14, 23, 24, 33, 34, 43, 44, 45 sleeve crown +15, 42 overdenture mixed support Occlusal reconstruction.Results: After the prosthesis was worn for 1 month, the patient’s occlusion function was restored, the pronunciation was clear, the face was improved, and the mental condition was good. Patient feel satisfied.Conclusion: The combined application of the telescopic crown and the overdenture provides good retention and stabilization of the removable partial denture while protecting the abutment effectively.更多还原
