与X线胸片对比分析肺超声在基层新生儿肺炎诊断中价值

【摘要】 目的与X线胸片对比分析肺超声(lung ultrasonography LUS)在基层新生儿肺炎诊断中的应用价值。方法:回顾分析了我院2014年7月至2015年11月,51例经临床诊断、肺超声(LUS)及X线胸片检查病历资料。其中男28例,女23例;最大年龄28d,最小年龄2d,平均年龄11.5d。出生时最大体重3920g,最小体重2240g,平均体重3155g。将肺超声(LUS)检查结果与临床诊断、X线胸片检查结果作对比研究分析。结果:51例中经肺超声(LUS)检查阳性35例,阳性率68.63%(35/51),阴性16例,占31.37%(16/51)。51例中X线胸片检查阳性33例,阳性率64.71%(33/51),阴性18例,占35.39%(18/51)。以X线胸片检查结果作为对比:51例中肺超声(LUS)检查出阳性32例,阴性15例,共计47例,与X线检查对比完全一致。肺超声(LUS)35例阳性中与X线胸片检查结果对比,超声有3例阳性,X线胸片检查为阴性,超声误诊率为8.57%(3/35);超声阴性16例中1例X线胸片显示阳性,漏诊率为6.25%(1/16)。经统计学处理肺超声(LUS)和X线胸片检查结果的相关系数为0.827(P<0.001),差异具有统计学意义。超声诊断肺炎敏感性、特异性分别为96.97%、83.33%。结论:肺超声(LUS)在基层新生儿肺炎诊断中有重要应用价值,是一项值得在基层医院可推广适宜技术。
【Abstract】 Objective: To investigate the application of chest X-ray examination and lung ultrasonography(LUS) in the diagnosis of neonatal pneumonia.Methods: Retrospectively analysis of 51 patients between July 2014 and November 2015 in our hospital with clinical diagnosis, LUS and chest X-ray examination. 28 M, 23F; oldest 23 d, youngest 2d, average age 11.5d; heaviest birth weight(BW) 3920 g, lightest BW 2240 g, average BW 3155 g. Comparative study of LUS medical records with clinical diagnosis and chest X-ray examination. Result: Among 51 cases of LUS, positive rate was 68.63%(35/51), negative 31.37%(16/51); 51 cases of x-ray examination, the positive rate was 64.71%(33/51), negative 35.39%(18/51). Compared to chest x-ray examination results, in 51 cases of LUS, 32 were diagnosed positive, 15 in negative, a total of 47 cases, entirely consistent with the x-ray contrast. 3 of 35 cases with LUS positive was found negative in chest x-ray examination, misdiagnosis rate was 8.57%(3/35), Only 1 of 16 LUS showed positive in chest x-ray, misdiagnosis rate of 6.25%(1/16). Statistically analysis of LUS and chest x-ray examination showed correlation coefficient of 0.827(P<0.001), the difference was statistically significant. LUS diagnosis of pneumonia, sensitivity and specificity are 96.97% and 83.33% respectively.Conclusion: The application of LUS is important in diagnosis of primary neonatal pneumonia. It is worth to promote at the community level.
