【摘要】 目的:探讨水剂结合经腹超声在诊断贲门失弛缓症中的优势。方法:对经临床、钡餐造影、内镜检查确诊的10例贲门失弛缓症患者,设立10名正常人作为对照组,对2组受试者进行经腹超声饮水试验,比较两者的超声表现,重点观察贲门及下段食管经腹超声表现进行观察分析。结果:(1)10例正常人饮水试验前食管腔及贲门口闭合,饮水时贲门迅速开放,水流快速通过。(2)贲门失弛缓症患者饮水试验前食管下段已经开始扩张:饮水时连续观察5-10min,贲门始终不弛张、水流不能通过者6例,贲门间断性张开、水流间断通过者4例:饮水试验后30 min再检查时见全部患者的食管中仍滞留有水。(3)与正常组相比,贲门失弛缓症患者贲门及下段食管的宽度,长度和壁厚差异显著(P<0.01)。结论:经腹壁超声对食管贲门部管壁的层次结构及其厚度具有较好的分辨力,可对绝大多数的贲门失弛缓症做出明确诊断,可作为随访观察及筛检贲门失弛缓症的首选方法,值得临床推广运用。更多还原

【Abstract】 Objective:To explore the advantages of abdominal ultrasound in the diagnosis of achalasia.Methods:The clinical,barium meal,endoscopy diagnosed 10 cases of patients with achalasia,established 10 healthy control group,on two groups of subjects were drinking transabdominal ultrasound test,compare the sonographic appearance focus observation cardia and lower esophagus transabdominal sonographic findings were observed and analyzed.Results:(1) 10 cases of normal water before the test cavity and esophagus Ben door is closed,the water quickly open cardia,flow quickly through.(2) Loss before drinking achalasia cardia patients have begun to test the lower esophageal dilatation;when drinking continuous observation 5-10 min,cardia never relaxation,water can not pass in 6 patients,cardiac intermittent open,intermittent water flow through those four cases;see test 30 min after drinking all the time and then check the patient’s esophagus still have water retention.(3) Compared with the normal group,patients with achalasia cardia and width,length and thickness of the lower esophagus were significantly different(P <0.01).Conclusions:abdominal ultrasound esophageal cardia and the thickness of the wall of the hierarchy has better resolution,can the vast majority of achalasia make a clear diagnosis,follow-up observation and screening as achalasia The preferred method is worthy of clinical use.更多还原
