应用64层螺旋CT评价β受体阻滞剂的应用对左心功能的影响

【摘要】 目的研究64SCT(64层螺旋CT)冠状动脉造影时β受体阻滞剂的应用对左心室功能的影响。方法收集行64SCT冠状动脉造影(64SCTA)检查未发现冠状动脉狭窄被试178例,其中89例检查前未服用β受体阻滞剂(普萘洛尔),另外89例年龄和性别相匹配的患者(检查前心率>70次/分)服用β受体阻滞剂调整心率。记录用药前后心率、收缩压(SBP)和舒张压(DBP),普萘洛尔剂量。测定左心功能指标:EDV、ESV、EF。未服用和服用普萘洛尔两组左心功能结果应用配对t检验进行统计分析。结果未服用和服用普萘洛尔两组间的收缩压(SBP)和舒张压(DBP)的差异无统计学意义(分别为124±14和81±9mm Hg;121±13和79±10mm Hg)。EDV在这两组之间无显著性差异(分别为127.5±16.4ml和129.8±15.1ml),而ESV和EF在这两组之间的差异有统计学意义(分别为44.7±12.3ml和47.3±13.4ml,65.1±8.4%和62.0±7.8%),服用普萘洛尔组ESV不同程度的增加,而EF下降。结论应用β受体阻滞剂会使ESV增加,EF降低,而SBP、DBP和EDV的差异没有统计学意义;因此应用β受体阻滞剂个体所得的心功能指标并不是准确、可靠的。

【Abstract】 Objective To investigate the effects of propranolol on the left ventricular(LV) function during CT coronary angiography.Methods 178 subjectsunderwent 64 SCT coronary angiography(64SCTA) examination that showed no coronary artery stenosis.89 subjects did not take propranolol while another 89 age-and gender-matched subjects who had a heart rate above 70 beats were chosen to take propranolol.The heart rate,systolic blood pressure(SBP),and diastolic blood pressure(DBP) before and after the administration of propranolol and the dosage of propranolol were recorded.The left ventricular function were calculated by parameters of EDV,ESV and EF.The paired-t test was used to compare the left ventricular function between the two groups(not taking and taking propranolol).Results There were no significant difference in the SBP and DBP between the groups without and with propranolol medication(124±14mm Hg and 81±9mm Hg;121±13 mm Hg and 79±10mm Hg,respectively).The EDV showed no significant difference between the two groups,but the ESV and EF showed significant difference between the two groups(44.7±12.3ml and 47.3±13.4ml,65.1±8.4% and 62.0±7.8%,respectively).Conclusion Taking Propranolol increases ESV and decreases EF,while the SBP,DBP and EDV of the subjects are not changed.
