甲状腺功能正常的2型糖尿病患者血清TSH水平与代谢综合征的相关性探讨

【摘要】 目的:探讨甲状腺功能正常的2型糖尿病患者血清促甲状腺激素(TSH)水平与代谢综合征(MS)的相关性。方法:将430例住院的甲状腺功能正常的2型糖尿病患者根据有无MS分为MS组、非MS(NMS)组,比较两组血清TSH水平,并分析其与MS主要组成成分的相关性。结果:MS组血清TSH水平显著高于NMS组(P<0.05)。按照血清TSH水平四分位法分组后,高TSH组2型糖尿病患者收缩压(SBP)、舒张压(DBP)、体质指数(BMI)、血清甘油三脂(TG)和MS患病率显著高于低TSH组,高密度脂蛋白-胆固醇(HDL-C)显著低于低TSH组(P<0.05)。Spearman相关分析显示血清TSH水平与BMI、SBP、DBP、TG呈显著正相关(P<0.05),与HDL-C呈显著负相关(P<0.05)。Logistic回归分析显示血清TSH水平与MS显著正相关(OR=1.308,95%CI:1.029~1.663)。结论:血清TSH可能亦是2型糖尿病患者MS的重要组成成分之一。

【Abstract】 Objective: To investigate the relationship between the Thyroid Stimulating Hormone(TSH) and Metabolic Syndrome(MS) in Type 2 Diabetes patients with normal thyroid function.Methods: 430 Type 2 Diabetes patients with normal thyroid function were divided into MS group and non-MS(NMS) group according to with or without MS.The Thyroid Stimulating Hormone(TSH) were measured and compared in two groups. The association between TSH and the components of MS was analyzed.Results: Compared to NMS group,the value of TSH was significantly increased in MS group( P<0.05). In accordance with the level of serum TSH quartile method, the values of systolic blood pressure(SBP), diastolic blood pressure(DBP), body mass index(BMI), triacylglycerol(TG),the prevalence of MS were significantly increased in the group of highter TSH than that of the lower TSH group, the value of high density lipoprotein(HDL-C) was significantly decreased in the highter TSH group than the lower TSH group(P<0.05). Spearman correlation analysis showed the positive association between TSH and BMI,SBP,DBP,TG(P<0.05), and the negative association between TSH and HDL-C(P<0.05). Logistic regression analysis showed the positive correlation between TSH and MS(OR=1.308,95%CI:1.029~1.663). Conclusion: TSH may be one of the components of MS in type 2 Diabetes patients.
