骨定量超声技术联合血清钙、磷、碱性磷酸酶测定评价早产儿代谢性骨病的初步研究

【摘要】 目的:探讨骨定量超声技术(QUS)联合血清钙、磷、碱性磷酸酶(AKP)测定评价早产儿代谢性骨病(MBD)的临床应用价值。方法:选择2011年9月至2013年3月入住我院NICU的适于胎龄的早产儿,住院时间4周以上、纠正胎龄达37~40周的早产儿16例为观察组;胎龄37~40周、日龄1周内的足月儿为对照组18例,分别测定血清钙、磷、碱性磷酸酶和左小腿胫骨内侧面骨超声传导速度值(SOS),并测定其身高、体重、头围。比较两组差异。结果:早产儿组血钙、磷、AKP和SOS值均明显低于足月儿组,差异有统计学意义(P<0.05),且两组AKP和SOS值有显著差异;早产儿组身高、体重、头围值均显著低于足月儿组,差异有统计学意义(P<0.01)。结果显示早产儿血钙、磷、AKP值和SOS值及至校正年龄足月时仍明显低于足月儿;早产儿代谢性骨病发病率仍较高。结论骨定量超声技术结合骨代谢生化指标可尽早识别MBD,定量超声技术有望成为早产儿代谢性骨病早期诊断手段之一。
【Abstract】 Objectives.The aim of this study was to investigate the clinical value of combine bone quantitative ultrasound technology(QUS)and the measurement of serum calcium(Ca),phosphate(Ph) and bone alkaline phospha-te(AKP) to assess metabolic bone disease in preterm infants.Methods.From the patients admitted to hospital during September 2011 to March 2013,Sixteen preterm infants(corrected gestational age to 37-40 weeks,hospitalized >4 weeks )were assigned to the observation group,eighteen fullterm infants(gestational age 37 to 40 weeks,age<l weeks)were assigned to the control group.we measured the serum Ca,Ph,AKP and the speed of ultrasound in left medial tibia(SOS).the height,weight,and head circumference of the patients in two group were re-ported.Results.The values of serum Ca,Ph and SOS in observation group were obviously lower than in control group,the difference has statistically significant(p<0.05).the Height,weight and head circumference of the infants in observation group were lower than in control group which aslo had statistically difference(p<0.01).the result indicated that the values of serum Ca,Ph and SOS in preterm infants who corrected age to fullterm were significantly lower than fullterm infants.the incident of metabolic bone disease in preterm infant was still high in hospital.Conclusions.The bone quantitative ultrasound technology(QUS) combine the measurement of biochemical markers of bone metabolism(Ca,Ph,AKP)could identify the metabolic bone disease early,the QUS could be the important method of early diagnosis of metabolic bone disease in pre-term infants.
