不同终末冲洗方案对环氧树脂类及硅烷类根管封闭剂与根管壁牙本质粘接性能的影响

【摘要】 目的:比较研究环氧树脂类根管封闭剂AH Plus及硅烷类根管封闭剂GuttaFlow在分别配合三种不同终末冲洗方案使用时与根管壁牙本质的粘接性能。方法:60颗单根直根管离体人牙截冠后根管预备,按照不同终末冲洗方案随机分为3组(n=20):组1、0.9%生理盐水(NS)对照组;组2、5.25%次氯酸钠(NaOCl)/17%乙二胺四乙酸(EDTA);组3、17%EDTA/2%氯己定(cHX)。使用螺旋充填器导入根管封闭剂,按照使用的根管封闭剂将每实验组分为两个亚组(n=10):组a.AH Plus;组b.GuttaFlow。每个牙根自冠方向根方依次获取3个厚度约为2mm的水平切片,以薄片推出实验检测根管封闭剂与根管壁牙本质的粘接强度,根管显微镜下观察断裂方式。结果:使用AH Plus根管充填时,EDTA/CHX组(5.37±2.80 MPa)粘接强度显著低于NaOCl/EDTA组(8.81±4.38 MPa)与NS组(9.07±2.77 MPa)(P<0.05),而NaOCl/EDTA组与NS组间无统计学差异(P>0.05)。使用GuttaFlow根管充填时,EDTA/CHX组(1.40±0.59 MPa)、NaOCl/EDTA组(1.26±0.62 MPa)、NS组(1.05±0.27 MPa)三组间均无统计学差异(P>0.05)。无论使用何种终末冲洗方案,AH Plus的粘接强度均显著高于GuttaFlow(P<0.05)。结论:GuttaFlow对牙本质的粘接强度显著低于AH Plus。17%EDTA/2%CHX作为终末冲洗方案在可显著降低AH Plus与根管壁牙本质的粘接强度,但对GuttaFlow的粘接性能无显著影响

【Abstract】 Objective:To evaluate the bond strength of epoxy resin-based sealers and silicone-based sealers to root canal walls with different final irrigation regimens.Methods:60 fresh extracted single-rooted teeth prepared chemomechanically using 5.25%sodium hypochlorite(NaOCl) as root canal irrigants were randomly distributed into three groups(n=20) according to the final irrigation protocol:group(1) control,0.9%normal saline(NS);group(2)5.25%NaOCl followed by 17%EDTA;and group(3) 17%EDTA followed by 2%chlorhexidine(CHX).Then filled the root canals with sealers using a lentulo spiral and each group was distributed into two subgroups according to the sealers(n=10):subgroup(a) AH Plus;subgroup(b) GuttaFlow.Each root was transversally sectioned from crown to apex to obtain three 2mm thick dentinal slices successively.Then a push-out bond strength test was carried out and failure mode was determined at x 24 magnification.Results:When filled the root canals with AH Plus,the push-out bond strength ofNS group(9.07±2.77 MPa) and NaOCl/EDTA group(8.81±4.38 MPa) were similar and significantly higher than EDTA/CHX group(5.37±2.80 MPa)(P<0.05).When filled the root canals with GuttaFlow,there was no significant difference among three irrigantion groups.AH Plus had a significantly higher bond strength than GuttaFlow regardless of the final irrigation regimens.Conclusion:The bond strength of AH Plus was significandy higher than GuttaFlow.17%EDTA/2%CHX final irrigation regimen could reduce the bond strength of AH Plus significandy while no marked effect to GuttaFlow.
