【摘要】 目的了解天津市2型糖尿病参保患者人年均经济负担并探讨其影响因素。方法通过问卷调查收集参保2型糖尿病患者的直接医疗费用、直接非医疗费用及间接费用。采用多元线性回归分析,对各种费用的影响因素进行研究。结果天津市参保2型糖尿病医保患者的人年均直接医疗费用、直接非医疗费用和间接费用分别为32997元、1727元和1595元。推算得天津市2型糖尿病总医疗费用为37.4亿元,占天津卫生总费用的6.22%,直接医疗费用和间接费用分别为1.95亿元和1300万元。多元回归分析结果表明,并发症个数、糖尿病史、患者类别、性别和文化程度是影响直接医疗费用的重要因素,工作状态、职业、患者类别、年龄和性别是影响间接费用的重要因素。结论天津市参保2型糖尿病患者经济负担沉重,给患者及卫生体系均带来较大影响。更多还原

【Abstract】 Objective To examine the annual economic burden of type 2 diabetes mellitus(T2DM) and to explore main factors associated with the economic burden among Tianjin insuredpopulation.Methods Cluster sampling for cases collected consecutively during the study periodwas administrated.The direct medical cost,direct non-medical cost and indirect cost werecollected with a structured questionnaire by well trained professional investigators.Multiple linearstep wise regression models were conducted to determine the factors linked to the economicburden.Results The average annual direct medical cost,direct non-medical cost and indirectcost were 32 997 Yuan、1 727 Yuan and 1 595 Yuan for each patient.The total direct medical costfor T2DM was 3.74 billion,which is 6.22% of total healthcare expenditure of Tianjin in 2011.Thetotal direct non-medical cost and indirect cost was 195 million and 13 million.Based on themultiple stepwise regression analysis,complication numbers,diabetes history,patient type,genderand education were the influencing factors to direct medical cost.Working status,career,patienttype,age and gender were influencing factors to indirect cost.Conclusion The annual economicburden was substantially high for T2DM patients and public health sector in Tianjin.更多还原
