【摘要】 目的探讨网织血红蛋白含量(Ret-He)对缺铁性贫血的临床诊断价值。方法选取经骨穿等方法确诊的缺铁性贫血(IDA)病例54例,非IDA贫血病例50例,正常对照组61例,采集所有对象2 ml乙二胺四乙酸二钾(EDTA-K2)抗凝静脉血,利用Sysmex XE-2100五分类自动血细胞分析仪检测其网织血红蛋白含量(Ret-He)及红细胞平均体积(MCV)、红细胞平均血红蛋白含量(MCH)、红细胞平均血红蛋白浓度(MCHC)、红细胞分布宽度(RDW-CV)、血清铁(SI)、血清铁蛋白(SF)含量,然后利用t检验分析三组间各参数间差异,并采用ROC曲线评价RET-He对IDA诊断的敏感度和特异性,确定其诊断IDA的cutoff值。结果 IDA组Ret-He、MCH、MCV、SF与其他两组相比差异均有显著性(t=13.751、-11.671、17.563、-15.725、14.630、-14.114、8.541、-6.919,P均<0.001)。IDA组MCHC、RDW-CV、SI三个参数与其他两组比,差异均有显著(P<0.001),非IDA贫血组MCHC、RDW-CV、SI三个参数和正常对照组比较,差异也有显著性(P<0.05)。ROC曲线评价RET-He对IDA诊断的临界值点为29.000pg时,灵敏度为90.7%,特异度为88.3%。结论 Ret-He对于IDA有很高的诊断效率,可以作为IDA的诊断指标。更多还原

【Abstract】 objective to explore the value of reticulocyte hemoglobin content in the diagnosing iron deficiency anemia.Methods:the methods of bone wear confirmed cases of iron deficiency anemia(IDA), the IDA anemia 54 cases,the no iron deficiency anemia 50 cases,cases of normal control group in 61 patients,collect all objects 2 ml ethylenediamine tetraacetate two k(EDTA - K2),using Sysmex XE anticoagulant venous blood 2100 five classification automatic blood cells - test their Ret-He and average volume(red red MCV),average hemoglobin content(MCH),average hemoglobin concentration (red MCHC),red blood cell distribution width(RDW-CV) - serum iron(SI),serum ferritin (SF) content,then use t-test analysis between three groups between parameters,and uses the ROC curves differences of IDA Ret-He evaluation of sensitivity and specificity,diagnostic determine the cutoff value.Results the Ret-He,of MCH,MCV,SF of the IDA group than the other two groups was significant difference(t= 13.751、-11.671、17.563、-15.725、14.630、-14.114、8.541、-6.919、P<0.001). MCHC,RDW-CV,SI of the IDA group compared with the other two groups,the difference is significant(P < 0.001),the MCHC,RDW-CV、SI of the no iron deficiency anemia group compared with the normal controls group,the differences were also significant(P < 0.05).when the cutoff is29.000,sensitivity and the specific degrees for 90.7%for 88.3%.Conclusion Ret - He has very high for IDA diagnostic efficiency,can be used as the diagnosis of IDA indicators.更多还原
