温州地区3岁以下住院儿童社区获得性肺炎病毒病原学研究

【摘要】 目的探讨温州地区3岁以下住院患儿社区获得性肺炎(community-acquired pneumonia,CAP)病毒病原学感染状况,为临床合理用药提供依据。方法 2010.2-2011.1在温州医学院附属育英儿童医院儿科确诊为CAP的3岁以下住院患儿,入院当天或次日清晨取其鼻咽部分泌物,采用直接免疫荧光法分别检测呼吸道合胞病毒(RSV)、腺病毒(ADV)、流感病毒A、B型(IVA、IVB)和副流感病毒Ⅰ、Ⅱ、Ⅲ型(PIVⅠ、PIVⅡ、PIVⅢ)7项呼吸道病毒抗原。结果 2031例标本中,阳性807例,阳性率为39.73%。其中RSV618例,占76.58%;PIVⅢ92例,占11.40%;IVA42例,占5.20%;PIVI20例,占2.48%;ADV14例,占1.74%;IVB4例,占0.50%;混合感染17例,占2.10%。结论 2010—2011年度温州地区3岁以下住院患儿CAP病毒病原以RSV为主,其次为PIVⅢ,IVA和PIVⅠ;病毒流行高峰为冬春季节;RSV的阳性率从11月开始升高(24.36%),1、2月达高峰(分别为51.76%,50.40%),持续到次年4月(19.63%),以后逐渐下降,6、7月最低(分别为1.15%,1.19%)。婴幼儿是病毒性肺炎防治的重点。更多还原

【Abstract】 Objective:To investigate the viral etiology of inpatient children below 3 years old with community acquired pneumonia(CAP) in Wenzhou.Methods:Children due to CAP less than 3 years old admitted to Yu Ying Children’ s Hospital Affiliated to Wenzhou Medical College between February 2010 and January 2011 were enrolled into the study.Their nasopharyngeal secretions(NPS) were obtained as soon as possible(usually within 24h) for identification of seven kinds of respiratory viral antigens by direct immunofluorescence assay.Results: Among the all 2031 samples,807(39.73%) positive results were determined.0f the 807,Respiratory syncytial virus(RSV) accounted for 618(76.58%),parainfluenzaⅢ(PIVⅢ) 92(11.40%),influenza virus A(IVA) 42 (5.20%),parainfluenza(?)(PIVI)20(2.48%),adenoviruses(ADV) 14(1.74%),influenza virus B 4(0.50%) ,and mixed infection 17(2.10%).Conclusion:RSV infections was dominant in inpatient children below 3 years old with CAP in Wenzhou,followed by PⅣⅢ.,ⅣA,PIⅥ.The peak season for RSV infection was spring, and winter(January to April or November to December)。Their was a higher incidence of virus infection in infant。The viral etiology of inpatient children below 3 years old with community acquired pneumonia in Wenzhou WANG Feng-mei,LUO Yun-chun(Yu Ying Children’s Hospital Affiliated to Wenzhou Medical College, Wenzhou 325000,Zhejiang,China)更多还原
