【摘要】 观察采用大黄辅助早期肠内营养(EEN)治疗重症急性胰腺炎(SAP)的疗效。方法回顾分析符合要求的127例SAP病人,分为两组:即大黄辅助EEN(REN)组37例和中期肠内营养(MEN)组90例。两组的观察项目:定量检测C-反应蛋白(CRP)和血清前白蛋白(PA),记录两组病人肠道功能恢复时间、器官损害数、感染部位数、胰腺假性囊肿数、病死率、住院时间、住院费用和APACHEⅡ评分等。结果治疗12～14天后,REN组死亡2例(5.4%),MEN组死亡14例(15.6%,)REN组与MEN组比较,APACHEⅡ评分和CRP明显下降,而PA明显升高(P<0.05);REN组病人的肠功能恢复时间、器官损害数、感染部位数、胰腺假性囊肿数、病死率、住院时间、住院费用均显著低于MEN组(P<0.05)。结论大黄联合EEN治疗SAP,能促进病人肠功能恢复,改善病人的营养状况,减少并发症,缩短住院时间,降低医疗费用。更多还原

【Abstract】 Objective To observe the effects of rhubarb assisted early enteral nutrition on severe acute pancreatitis(SAP).Methods One hundred twenty seven patients with SAP from January 2005 to December 2011 were divided into two groups:37 patients were treated with rhubarb assisted early enteral nutrition(Group REN) in 2-5 day after occurred SAP,and 90 patients treated with median enteral nutrition(Group MEN) in 7-10 day occurred SAP.The level of C-reactive protein(CRP) and prealbumin(PA) were detected,The recovery of intestinal gurgling sound,the number of organ injury,infective position and pancreatic pseudcyst,mortality,length of stay,the hospitalization cost and APACHEⅡwere observed.Results:There was 2 patients death in REN group,and 14 patients death in MEN group.After 12-14 days therapy,in group REN the score of APACHEⅡand CRP,the recovery of intestinal gurgling sound,the number of organ injury, infective positions and pancreatic pseudcyst,mortality,length of stay,and the hospitalization cost significantly reduced,while PA significantly ascended Compared with group MEN.Conclusion:The therapy of rhubarb assisted early enteral nutrition can make the complications,mortality,length of stay and the hospitalization cost significantly reduce in SAP.更多还原
