125I放射性粒子植入术后患者周围辐射剂量的监测与防护

【摘要】 目的:探讨125 I放射性粒子植入术后患者周围辐射剂量的监测与防护。方法:随机抽取2004年10月~2005年12月在中山大学附属肿瘤医院在CT引导下行125I放射性粒子植入治疗恶性肿瘤患者20例,用γ射线监测仪在术后即时及2、4、6个月后对125I放射性粒子辐射剂量进行监测,将监测结果与室内天然本底剂量进行比较。结果:随着与放射源距离的增大,辐射剂量迅速减少所有病例在距离为50cm时,测得的剂量已接近室内天然本底辐射剂量;随粒子植入体内的深度越深,在体表0cm处辐射剂量越接近天然本底剂量;在植入深度相同的情况下,植入的粒子数量越多,测得的辐射剂量越大;辐射剂量随着时间的推移而递减,在6个月后体表0cm处测得的剂量已基本接近室内天然本底剂量。结论:在125I放射性粒子植入术前后对辐射的防护除应积极采取隔离防护措施以外,同时应注意距离防护、时间防护以及根据需要严格控制粒子数量。只要严格采取了以上必要的防护措施,125I放射性粒子植入治疗对于病人本人及周围人群是安全的。
【Abstract】 OBJECTIVE:To investigate the radiation dose detection and protection after 125I radioactive seed implantation.METHODS:From Oct 2004 to Dec 2005,20 malignant tumor cases was randomized selected in this study after 125I radioactive seed implantation and radiation dose was detected immediately after the operation and 2,4 and 6 months later with y-ray equipment.RESULTS:With the increase of the distance from radioactive source,the radiation dose decrease quickly;The detected dose were nearly the same with natural background radiation dose at a distance of 50cm from the patients in all cases.Deeper the seed implanted,the radiation dose more near to natural background radiation dose.The more seeds were implantation and the radiation dose was higher at the same depth from the seeds implanted.With time lasting,the environment radiation dose was attenuated and the detected dose was same with natural background after 6 months,CONCLUSION:Addition to the isolation and shielding protection before and after the seeds implantation,distance protection and time protection should be considered.Strictly protect methods was necessary after received this procedure and it is safe to patients and environment.
