【摘要】 目的介绍应用Burow’s楔形推进皮瓣修复面部皮肤缺损的经验与体会。方法在遵循肿瘤切除的原则和面部分区修复的基础上,应用Burow’s楔形推进皮瓣联合固定技术修复面部肿瘤切除后皮肤软组织缺损创面,并使辅助切口顺应面部分区自然轮廓线(鼻唇沟、鼻翼沟、颌鼻缝、睑缘、唇红缘、发际等)或皮肤皱纹处,尽可能隐藏切口线。结果共施行手术84例,面部缺损面积最小者约为1.0cm×2.0cm,最大者约为3.5cm×6.0cm。术后皮瓣全部成活,无明显并发症发生。其中61例获得随访1～50个月,修复组织与周围皮肤组织在色泽、质地、轮廓等方面相似、匹配良好,且未见肿瘤病例复发,切口线相对隐蔽、切口瘢痕平软,面部外形轮廓及视觉效果良好,均获得满意面部形态和美学效果。结论利用Burow’s楔形推进皮瓣修复面部皮肤缺损创面,辅助切口少且切口相对隐蔽,同时皮瓣转移后较自然平滑过渡,创面修复后外形结构特征更为自然,可获得满意面部形态结构和美容重建效果。更多还原

【Abstract】 Objective To introduce the experience and comprehend of application of Burow’s wedge advanced skin flap in repair of defect of the face.Method In base of following the tumor excision and zonation repair in face,we repaired face skin deficiency followed face tumor and scar excision by applying Burow’s wedge advainced skin flap, and made the aid incision accommodate to natural contour line(nasolabial groove,sulcus alaris,maxillonasal suture, lid-margin,vermilion border,hairline and so on) or skin wrinkle,and hided the incision.Result We performed operations for 84 examples,the minimum area of face deficiency was 1.0cm×2.0cm,the maximum was 3.5cm×6.0cm.The flap all survived postoperatively without obvious complications.61 examples among 84 were followed up for 1 to 50 months,repaired tissue matched well with surrounding tissue at color,texture,contour and so on, tumor cases didn’t recurred,incision line was hided relatively,incision scar became smooth and soft,face contour and visual effect were well,all examples obtained satisfacted face shape and aesthetics result.Conclusion In using Burow ’s wedge advanced skin flap to repair face skin deficiency,aid incision was few and incision was concealed relatively,the flap was transited naturally and smoothly when transferred,structural feature of contour was more natural after wounded surface was repaired,satisfacted face appearance and cosmetic reconstruction effect was obtained.更多还原
