造影剂碘流率对肺结核患者MSCT肺动脉成像的理论探讨

【摘要】 目的探讨碘流率(单位时间内的碘流量)对肺结核患者MSCT肺动脉成像的影响,以指导对比剂的优化使用。方法 40例行胸部CT增强扫描的肺结核患者随机分为两组(每组各2 0例),A组采用300mgI/mL对比剂,剂量80mL,注射速率4.0mL/s;B组采用400mgI/mL对比剂,剂量60mL,注射速率3.0mL/s,追加生理盐水20mL。行胸部肺动脉同层动态扫描,根据时间-密度曲线(TDC),计算肺动脉的强化峰值(PV)、峰值时间(PT)以及强化CT值大于200Hu的起始时间(Tb200)和持续时间(T200)。结果肺动脉的强化峰值,A组和B组分别为(372.45±58.60)Hu和(398.45±84.80)Hu,两组无显著性差异;而峰值时间、Tb200及T200,两组均存在显著性差异。结论对比剂应用中,碘流率是影响MSCT肺动脉成像图像质量的关键因素;使用高浓度对比剂时,适当减少剂量、降低注射速率不会影响图像质量。

【Abstract】 Objective To investigate the effect of iodine flow rate(iodine flow quantity in unit time) on multi-slice computed tomography(CT) pulmonary angiography,and to guide the optimal use of contrast material.Methods Forty patients randomly were divided into two groups(group A and group B,each group including 20 patients).The patients of group A were administrated intravenously 80mL of 300mgI/mL at a flow rate of 4.0mL/s,and group B were 60mL of 400mgI/mL at 3.0mL/s followed by 20mL saline at the same flow rate.All underwent dynamic scanning at the same pulmonary artery level.The pulmonary arterial peak time(PT),peak value(PV),the beginning time of more than 200Hu(Tb200) and duration time(T200) were analyzed.Results The peak value of pulmonary artery of group A and group B were(372.45±58.60) Hu and(398.45±84.80) Hu,respectively.There was no significant difference between them.To peak time,Tb200 and T200,there was significant difference between group A and group B.Conclusion Iodine flow rate is the critical factor which can affect the image quality of CT pulmonary angiography;Using higher concentration contrast material,the dose and injected rate should be decreased appropriately,and the image quality was not weaken.
