外源性色素渗入牙齿深度的实验研究
Study on the Penetration Depth of Extrinsic Tooth Stain in Vitro

【摘要】 目的:探讨外源性色素渗入离体牙内部形成内在性着色的深度及与着色时间和釉质表面粗糙度的关系,为完善牙着色模型的建立提供参考。方法:将牛牙制备成5mm×5mm×5mm的牙体组织。树脂包埋后开窗暴露釉质面和单侧纵剖面,其中釉质面为着色面,剖面是测量面。将21个牙体组织块随机分配至A,B,C三组,每组7个样本,A组为对照组,蒸馏水浸泡6天;B和C组分别为釉质未打磨组和打磨组,使用茶溶液浸泡6天。在浸泡过程中,釉质面需与茶着色溶液接触,测量面则需严格封闭。着色前和着色后1,2,3,4,5,6天,利用体视显微镜采集样本剖面图片并用Photoshop软件测量分析从近釉牙本质界到近髓腔处的3个不同深度牙本质区域(分别标记为Depth1,Depth2,Depth3)的△L,△a,△b和△E。使用Spss 13.0软件对结果进行统计分析。结果:测量面采集图片显示,A组无色素渗入情况,BC两组样本则可见明显色素渗入。A组各深度牙本质区域△E值随着着色时间的延长没有表现出明显的统计学变化,而BC组△E值随着色时间的延长而不断增加。在Depth1牙本质区域,BC两组△E值变化与A组之间存在显著差异。在Depth2区域,C组△E值变化与A组之间存在统计学差异,B组△E值变化与A、C组之间均无统计学差别。在Depth3区域,C组△E值变化与A、B组之间均存在显著差异,而AB组之△E值差异无统计学意义。结论:在本实验条件下,外源性色素能渗入到离体牙内部形成内在性着色,渗入深度能够达到近髓腔处的牙本质区域。着色深度与着色时间相关。打磨后釉质面更容易使色素渗入,这可能是因为打磨后釉质表面通透性增加而造成的。
【Abstract】 Objective:To study the penetration depth of extrinsic tooth stain in vitro and provide the reference for the standard staining model in the future.Methods:21 bovine teeth were distributed randomly into 3 groups with 7 blocks in each:Group A was the control one stained by distilled water;Group B was stained by tea without enamel surface burnished;Group C was stained by tea with enamel surface burnished.3 dentinal areas of each group were assessed,which were ’ Depth 1’ near the enamel-dentin junction, ’Depth3’ near the pulpal wall and ’Depth2’ between them.Color assessments for the 3 areas were made at baseline,1st,2nd,3rd,4th,5th and 6th day after staining by digital stereoscopic microscope and Photoshop software to get L,a,b values of CIE Lab system.△L,△a,△b and△E were calculated at each test time point.Results:In Group B and C,△E changed significantly with staining time(P<0.05 ),while no such change was found in Group A.In Depth 1,△E in Group B and C were significantly different from that of Group A.In Depth2,there was significant difference of△E between Group A and C,but no such difference was found in Group B when compared with Group A and C.In Depth3,△E of Group C was significantly different with both Group A and B,while Group A and B had no obvious difference.Conclusion:Chromogens can penetrate into the dentine through enamel surface in vitro and the penetration depth is related with staining time.Burnished enamel can make the penetration of chromogen more easily through the surface,which may affect the chromogen penetration.Therefore,the penetration of chromogen should be considered when establishing the standard staining mode in the future.
