利用CBCT评估中国人下颌磨牙根管形态
Use of CBCT to evaluate root and canal morphology of mandibular molars in Chinese individuals

【摘要】 目的:利用CBCT评估中国人下颌第一、二磨牙的根管形态。方法和材料:登记需要CBCT影像检查来辅助诊断和治疗的病人。389个健康未经过治疗,发育良好的下颌磨牙被纳入本实验。分别记录以下观察结果:(1)根的数目和它们的形态,(2)每个根的根管数量,(3)根管形态,(4)远中舌根在下颌第一磨牙出现的频率和(5)C形根管在下颌第二磨牙出现的频率。根据Vertucci的方法进行根管形态分类。结果大多数的下颌磨牙(70%的第一磨牙,76%的第二磨牙)有两个独立的根;29%的第一磨牙有远中舌根;C形根管出现在29%的第二磨牙上。56%的下颌第一磨牙发现了三个根管,43%具有四个根管。46%下颌第二磨牙具有三个根管,38%具有两个根管。大部分远中根是简单的Ⅰ类形态,近中根具有更复杂的根管系统,具有至少一个根管。下颌第一磨牙的根管形态有7种变型,下颌第二磨牙的根管形态有8种变型。结论:在中国人群中,拥有三个根的下颌第一磨牙和C形根管的下颌第二磨牙发生率较高。CBCT是探测可能存在的远中舌根和C形根管的有效工具,可以为根管治疗提供有力帮助。
【Abstract】 Aim To evaluate the root canal configuration of permanent mandibular first and second molars in a Chinese subpopulation using cone-beam computed tomography (CBCT).and materials Patients who required CBCT radiographic examinations as part of their routine examination,diagnosis,and treatment planning,were enrolled.A total of 389 healthy,untreated,well-developed mandibular molars were selected for this investigation. The following observations were recorded:(1)the number of roots and their morphology;(2) the number of canals per root;(3)the canal configuration;(4)the frequency of distolingual roots in the mandibular first molars;and(5)the frequency of C-shaped canals in the mandibular second molars.The root canal configurations were classified according to Vertucci’s method.Results The majority of mandibular molars(70%of first molars,76% of second molars)had two separate roots;however,three roots were identified in 29% of first molars.C-shaped roots occurred in 29%of second molars.Three canals were found in 56%of mandibular first molars,and 43%had four canals.In the mandibular second molars,46%had three canals and 38%had two canals.Most distal roots had a simple type I configuration,whereas mesial roots had more complex canal systems,with more than one canal.There were seven variants of the root canal morphology among the mandibular first molars and eight variants among the mandibular second molars. Conclusions Three-rooted mandibular first molars and C-shaped mandibular second molars were highly prevalent in this Chinese population.CBCT is an effective tool for the detection of additional distolingual roots and C-shaped roots/canals,and is a valuable aid for dentists providing root canal treatment.
