南通市成人白血病危险因素的1:2病例对照研究

【摘要】 目的探讨南通地区引起成人白血病的可疑危险因素,为预防白血病的发生提供科学依据。方法采用以医院为基础的1:2匹配设计病例对照研究方法 ,对2007年12月~2008年12月就诊于南通市第一人民医院和南通大学第一附属医院的69例确诊白血病病人及138例对照病人进行回顾性调查。每例病人以匹配条件选择2名非血液病病人作对照。对所有的病例和对照采用统一问卷调查。调查受试者的一般情况、吸烟饮酒史、家庭周围居住环境、居室装修史、有害物质接触史、疾病史和家族遗传史等,应用条件Logistic回归模型对资料进行分析。分析过程遵循先单因素后多因素的原则,对单因素分析结果有意义(P<0.2)的变量再进行多因素的逐步回归分析。结果单因素Logistic回归分析显示,白血病的危险因素有:肥胖、经常吸烟、住所周围有"三废污染"、农药接触史、苯接触史、其他有机溶剂接触史、电磁辐射、居室装修史、家族癌症史。多因素Logistic回归分析显示,危险因素为:住所周围有"三废污染"、农药接触史、苯及其他有机溶剂接触史、电磁辐射、家庭装修史。结论住所周围有"三废污染"、接触农药、苯和其他有机溶剂、暴露于电磁辐射、家庭装修后<3个月入住可能是南通地区成人白血病的主要危险因素,白血病的发生与多种因素相关,防治工作应从多方面立体展开。

【Abstract】 Objective To identify risk factors of leukemia and provide scientific basis for preventing the development of leukemia.Methods A hospital-based 1:2 case-control study on risk factors of leukemia was conducted in the First Affiliated Hospital of Nantong University and the First People’ s Hospital of Nantong between Dec.2007 and Dec.2008.69 leukemia patients and 138 patients were selected as cases and controls,respectively. The matched controls were non-hematopathy patients from the two hospitals.All information was collected by the same questionnaire.Investigations included general information,smoking and drinking history, outdoor environment around the residence,house decoration history and duration of moving in,toxicant contact history,disease history,family history of genetic diseases.Based on univariate Logistic regression analysis,a forward multivariate analysis was performed to select the significant variables.Results Univariate conditional Logistic regresson analysis showed that the following factors were significantly associated with the development of leukemia:fatness,smoking,residence surrounded by"three waste pollution",pesticide,benzene and other organic solvents,electromagnetic radiation,house decoration history,and family cancer history.Multivariable Logistic regression analysis showed that the following factors were significantly associated with leukemia:pesticide, benzene and other organic solvent,electromagnetic radiation,residence surrounded by"three waste pollution", house decoration history.Conclusions The main risk factors of leukemia were probably intimate contacting with pesticide,benzene and other organic solvents,exposed to electromagnetic radiation,residence surrounded by"three wastes pollution"and house decoration history.Preventive work should be done in many ways.
