【摘要】 目的根管再治疗很难避免或预测术后的疼痛和肿胀,通过根管器械和根管糊剂的不同组合,评价其对根管再治疗后疼痛反应的影响。方法:选择根管再治疗66颗患牙,随机分配为两组:ProTaper镍钛机动器械组(以下称机动ProTaper组)36例,手用不锈钢k型锉组(以下称手用k锉组)30例。根据Gerogopoulou标准分级,比较治疗期间各组疼痛反映情况。再分别将机动ProTaper组及手用k锉组各随机分为Cortisomol糊剂组和氧化锌丁香油糊剂组(以下称ZOE组),即:机动ProTaper+Cortisomol组18例,机动ProTaper+ZOE组18例,手动k锉+Cortisomol组15例,手用k锉+ZOE组15例,以VAS标尺法对术后,2天及7天的疼痛进行主观评价。结果:1、机动ProTaper组与手用k锉组比较,术中疼痛发生率经x~2检验两者有显著性差异。机动porTapor组比手用k锉组治疗期间疼痛发生率低。2、四种组合无痛、2天自愈及7天自愈的发生率经x~2检验有显著性差异。经析因相关分析,机动ProTaper+Cortisomol组根充后疼痛发生率最低持续时间最短,手用k锉+ZOE组疼痛发生率最高时间最长。此外,Cortisomol糊剂较ProTaper机动器械对无痛反应的影响大。结论:ProTaper机动镍钛器械结合Cortisomol根充糊剂可明显减少根管再治疗期间及术后的疼痛反应,为临床常用器械及糊剂较好的组合。更多还原

【Abstract】 OBJECTIVE::To evaluate the effects of the units of different instruments and different fill materials on the pain reaction of endodontie retreatment in order to seek for a better unit style for clinical retreatment.METHODS:A total of 66 teeth for retreatment was randomly divided into two group:the ProTaper rotary NiTi instrument group with 36 samples and the stainless steal hand-k files with 30 samples.The pain reaction during the retreatment were evaluate by Gerogopoulou criterion.Then each group were divided into two group:Cortismol past group and ZOE group.The occurrence rates of no pain,recovery with in 2 days and 7days were evaluated among the four groups by VAS criterion.RESULTS:1、x~2 test was performed for the rates of pain occurrence in ProTaper rotary instrument group and hand-k file group,there was significant difference,P<0.05.The rate of pain occurrence in ProTaper instrument group is lower.2、x~2 test was performed for the occurrence rates of no pain,recovering within 2days and 7 days in four different unit groups.There are significant different respectively.By factorial analyze,the pain occurrence rate was lowest and the time of pain was shortest in the ProTaper rotary instrument and Cortisomol group.The pain occurrence rate was highest and the time of pain was longest in the hand-k file and ZOE group.In addition,the role of Cortisomol paste to no pain reaction after the endodontic retreatment was better than the ProTaper rotary NiTi instrument.CONCLSION:Applying the unit of ProTaper rotary instrument and Cortisomol paste can decrease the rate of pain occurrence during endodontic retreatment and after the retreatment. The unit is better style among common clinical root canal therapy instruments and pastes.更多还原
