【摘要】 目的:对儿童早期窝沟龋进行窝沟封闭,使用龋齿诊断仪(DIAGNOdent)监测封闭后龋损进展情况,客观评价窝沟封闭对早期窝沟龋的抑龋作用,探讨激光荧光龋齿诊断仪应用于早期窝沟龋临床诊断的可行性。方法:对139名6-9小学生的289颗第一恒磨牙进行了视诊和激光荧光龋齿诊断仪(DIAGNOdent,DD)检查,视诊检查参照Ekstrand视诊分度系统,记录视诊指数,分为不同程度早期龋损组,对视诊指数为0-2°的儿童,使用透明窝沟封闭剂(Fissturit,Voco,Germany)进行封闭,封闭前后使用DIAGNOdent测定DD值。6个月和10个月后复查,检查封闭剂保留情况,再使用DIAGNOdent测定DD值,观察窝沟封闭后早期龋损的进展情况。结果:使用DIAGNOdent检测窝沟封闭前后DD值,经配对t检验,无统计学差异(p>0.05)。窝沟封闭6个月后,不同视诊组DD值均有下降,与基线值有显著性差异(p<0.01);视诊2°组,10个月后DD值仍有下降,其他组无明显变化。视诊分度与DD值的Spearman相关系数为0.811。结论:窝沟封闭可预防和抑制早期窝沟龋,使其趋于静止;透明窝沟封闭剂不影响DIAGNOdent对窝沟早期龋的检测;DIAGNOdent龋齿诊断仪检测与视诊检查有较好的相关性,可辅助视诊进行早期窝沟龋诊断,有助于纵向监测窝沟早期龋损的进展。更多还原

【Abstract】 Objective:To evaluate the use of DIAGNOdent for monitoring the progression of initial occlusal caries after fissure sealing.Methods:139 children aged 6-9 years participated in the study.289 permanent molars were examined visually and using DIAGNOdent(DD) by one dentist.The clear fissure sealants(Fissurit,Voco,Germany) were applied and then measured by DD. After 6 months and 10 months,all students were recalled and the DD were re-measured.Results:After 6 months of fissure sealings, the mean DIAGNOdent value was significantly decreased than that of the baseline in different groups(p<0.01).The mean of DIAGNOdent value at baseline was positively correlated with visual score.(r=0.811).Conclusion:Fissure sealant can inhibit the progression of initial occlusal caries.DIAGNOdent is useful in monitoring the development of initial occlusal caries in permanent molars and may be useful as an adjunct to visual examination in routine dental check-up.更多还原
