【摘要】 背景奥沙利铂已被广泛应用于结直肠癌的治疗,周围神经毒性是其剂量限制性毒性,主要表现为手足麻木、疼痛甚者出现肢体功能障碍,影响患者生活质量及化疗的顺利进行,目前尚缺乏明确有效的治疗方法。目的观察温经通络法外用治疗化疗致周围神经毒性的临床疗效和安全性。方法选取使用奥沙利铂化疗后出现2级以上周围神经毒性不良反应者为观察对象,随机分为2组,中药组采用温经通络法外用(洗/浸),对照组使用温水,连用7天为1个观察周期。疗效评价方法:参照NCI周围神经毒性分级标准比较治疗前、后的症状和体征,用药后降至0级为治愈、较用药前下降1级以上为有效、无改善为无效。结果中药组30例患者治疗后手足麻木的症状明显减轻、临床症状持续时间明显缩短,有效率93.3%(28/30)、其中治愈率83.3%(25/30),达到"有效"评价标准的起效时间为(5.7±3.2)天;对照组30例患者有2例患者中途退出,总有效率32.1%(9/28),其中治愈率21.4%(6/28),P<0.01。结论温经通络法外用可有效治疗化疗致周围神经毒性能够减轻症状、提高患者生活质量,且使用安全。更多还原

【Abstract】 Background:Oxaliplatin is increasingly used in the treatment of colorectal cancer,and chemotherapy-induced peripheral neuropathy(CIPN) is a common side effect.CIPN can cause numbness,tingling,shooting/burning pain and functional limitations,it may has an impact on patients’ quality of life (QoL) and the treatment plan.Effective and acceptable methods have been explored until now with certain difficulties.Objective:To evaluate the efficacy and safety of external use of Chinese herbs in CIPN caused by Oxaliplatin.Methods:Patients with peripheral neuropathy grade >1 (National Cancer Institute- NCI) during or after oxaliplatin therapy.Herbal Group:compound Chinese herbal infusion was used to soak the feet and hands,30min,Bid,7 days as a cycle.Control Group:warm water was used as the same way.Curative:Grade 0 or symptoms disappear completely.Effective:Grade decreases more than 1 level.Total efficiency:effective and curative.Results:In the Herbal Group,30 patients were enrolled,28 patients reported symptom relief without any side effect as tetter or allergy.Total effective rate of all patients was 93.3% (28/30),curative rate 83.3% (25/30).The average time from therapy beginning to effect is 5.7±3.2 days.In the Control Group,2 patients dropped out,Total effective rate of all patients was 32.1% (9/28),curative rate 21.4% (6/28).(P<0.01) Conclusion:The external use of Chinese Herbs could treat CIPN caused by oxaliplatin,which is effective and convenient in clinical without any side effect.Thus improve quality of life and help chemotherapy progress.更多还原
