【摘要】 目的通过外周血淋巴细胞亚群在人类同种异体肾移植受者术后感染的动态变化,探讨其与感染类型及愈后的关系。方法采用流式细胞仪,测定16例肾移植受者术后并发感染时外周血中CD3~+、CD4~+、CD8~+细胞,并根据CD4~+细胞计数及CD4~+/CD8~+比值,分析其与感染类型和愈后的关系。结果(1)、机会感染,其外周血CD4~+细胞计数明显低于普通感染组和术前(P<0.05),CD4~+ /CD8~+比值亦显著降低(P<0.05)。而普通感染CD4~+细胞计数和CD4~+/CD8~+比例多在正常范围,且与术前相比较,差异不显著(P>0.05)。(2)、CD4~+细胞计数和/或CD4~+/CD8~+比值明显降低者,往往预后较差。结论根据外周血淋巴细胞亚群的动态变化,特别是CD4~+细胞计数及CD4~+/CD8~+比值,可以初步判断肾移植受者感染类型及预后,并可指导临床合理用药。更多还原

【Abstract】 Objective:To investigate the relationship between the clinical significance of the dynamic changes in T lymphocyte subsets of peripheral blood and types of infection in transplant recipients,the prognoses was also anaylyzed.Methods: Sixteen cadaveric renal transplant recipients were enrolled in this study.T lymphocyte subsets of peripheral blood were assayed with flow cytometry.Results:The difference of CD4~+ lymphocyte number and CD4~+/CD8~+ratios between ordinary infection and opportunism infection was significantly(P<0.05).The difference in opportunism infection between pre-infection and post-infection was also significantly(P<0.05).No apparent difference were found in ordinary infection between pre-infection and post-infection(P>0.05).CD4~+ lymphocyte number and CD4~+/CD8~+ratios in ordinary infection varied in normal ranges(P>0.05).Conclusions:These results indicates that monitoring of peripheral blood T lymphocyte subsets especially the CD4~+ lymphocyte number and CD4~+/CD8~+ ratio is valuable for the diagnosis,therapeutic regimen and prognostic analysis in kidney transplant recipients.更多还原
