急性心肌梗死发病时间生物学研究

【摘要】 目的探讨急性心肌梗死发病的时间规律,为时间预防和治疗急性心肌梗死提供理论依据。方法:收集山东省烟台毓璜顶医院1998-2006年9年中入院的急性心肌梗死患者病例1257例,男889例(62.2±12.2岁),女368例,(69.0±9.9岁)。以患者胸痛等症状出现作为发病时间,分别记录每例患者的发病时刻及日期。应用SPSS10.0分析比较统计数据。结果1)1257例急性心肌梗死患者中,男性发病明显多于女性(7:3),男性患者的平均年龄明显小于女性,平均相差6.8岁(P<0.01);2)急性心肌梗死发病时间呈现明显的昼夜与季节性变化(P<0.01),昼夜高峰在6:01-8:00(17.9%),最低在0:01-2:00(4.5%)(P<0.001);季节高峰见于冬季(32.1%,最高见于1月),夏季(21.0%)较少,差异有显著性(P<0.01);3)未见发病的明显周节律性变化(P>0.05)。结论急性心肌梗死患者发病呈现明显昼夜与季节性变化,发病高峰分别在上午和冬季。
【Abstract】 Objective To explore the rhythmicity in the onset of acute myocardial infarction(AMI)and the result may provide evidence for the prevention and treatment of the patients with acute myocardial infarction.Methods 1)1257 hospitalized AMI patients entered into analysis from Jan,1st.2001 to Dec, 31th.2006 in Yuhuangding Hospital,Yantai.Among them,889 men,aged 62.2±12.2 yrs.and 368 women,aged 68.99±9.9 yrs.There were records of clear chest pain onset in 822 patients and they were involved in analysis of circadian variations.When the precise onset time could not be identified,only the date of onset was registered,The time records of chest pain attack were analyzed by circadian,circaseptan, circaseasonal or circannual variations.We register the exact time of the onset-the hour,the week,and themonth.We deal with the data with statistical method,and to check whether it has significant difference. Results 1)The number of male are more than the number of female in all subject(AMI),The average age of male are less than the female in AMI patients.2)The onset of AMI exhibit a circadian rhythm.38.9% of episodes occurred between 6:01-12:00,there is a significant difference in the occurrence among four 6-hour period of day,and 6:01-8:00 which formed a dominant morning peak,the trough was in night(p<0.01);3)Themonthly variation of AMI onset in total subjects tended to show one peak in winter (32.1%)(P<0.01).4)A weekly rhythm ofAMI could not be demonstrated(P>0.05).Conclusion The onset of AMI exhibit a circadian rhythm and a circaseasonal rhythm It showed a single peak in the period of 6:01-8:00 in the day and another peak in the winter in the year in the onset of all AMI cases.
