【摘要】 目的:使外阴癌病人能解除自卑心理,解除思想顾虑,积极配合,尽早手术,同时满足病人术后性生活和谐需要,提高生活质量。方法:对12例拟行外阴癌根治性切除和双侧腹股沟淋巴结清扫的病人,术前进行了心理护理,了解病人及其配偶对疾病的认识,收集她们共同关心的问题,针对病人家属对疾病的认识程度不同及担心术后性反应能力改变的问题,为病人家属提供相关知识;术前一周常规阴道冲洗,术后24小时内髋关节伸直位,术后病人外阴暴露护理, 术后24小时双下肢外展20度,屈髋20度,膝下垫软枕,房间温度保持24-16摄氏度,湿度60％左右,每日紫外线消毒2次,术后4～6天保持腹股沟持续负压吸引通畅,外阴局部采用电磁波治疗仪照射,切口局部用红外线热疗仪热疗, 常规留置导管5—7天,预防褥疮,术后第3、4、6、8周分别使用阴道扩张器进行阴道扩张。结果:12例外阴癌拟行手术的病人自卑心理消失,愉快接受手术,创口愈合1个月后, 12例病人均恢复正常性生活。更多还原

【Abstract】 Objective: To relieve the self-abased mental state and anxiety of the patient with vulva. To let the patient cooperate doctor treatment and meet the postoperative patient needs in sexual life. To improve patient life quality. Methods: 12 patients with vulva cancer were operated for thoroughly excising the focus and two-side lymph node in their inguen. The preoperative mental care was made to the patients. We investigated cognition and standpoint of the patients and their families for the disease. We collected their important viewpoint for the disease. In regard to the patient undersdanding degree for disease and worring degree for post-operative recover, we provided related medical knowledge for the patient and their families. Vulva was routinely scoured within a week before operation. Hip joint was extended, and the post-operative patient vulva was exposed, and both legs were extended out to 20?degree within 24 hours after operation. Then hip jonit was flexed to 20?degree. The soft pillow was put bellow knees. Tern- perature of the room is 24-16 ℃. Humidity is about 60%. The room was sterilized with ultraviolet ray twice everyday. The negative-press inhaler maintains in inguen in 4-6 days. Vulva skin was treated with electromagnetic wave. The incision was treated with infrared rey. The catheter was routinely reserved in 5-7days to prevent bed sore. The vulva was dilatated in third, fourth, sixth and eighth day after operation. Results: The self-abased mental state of 12 postoperative patients with vulva cancer disappeared, and they were happy.
