【摘要】 目的:探讨三维适形放疗(3DCRT)对宫颈癌的近期疗效及放疗并发症的发生率。方法:2001年1月至8月对我院收治的30例宫颈癌患者施行3-DCRT,所有病例放疗前行CT检查,用redplan3．0作出三维适形计划,靶区剂量DT45Gy,观察放疗近期疗效及消化道和膀胱的放疗反应。同期对照组50例同条件病例采用常规放疗技术,观察内容同3DCRT组。结果:三维调强适形放疗计划给肿瘤区提供较理想的靶区适合度,提高靶区剂量。使周围正常组织如膀胱、直肠得到有效的保护,患者对放疗的耐受性提高。 0-I级的消化道反应为83．33%。Ⅱ级消化道反应3DCRT 组为16．66％常规放疗组为26．00％(P<0．01)。0级的膀胱反应为86．67％。I级泌尿道反应为13．33％常规放疗组为16．00％(P>0．01)无显著性差异。两组病例近期有放率比较和一年生存率无明显差异,但两年生存率在统计上有明显差异(观察组为86．20％,对照组为77．08％P<0． 01)结论:3DCRT的放疗近期有效率等同于常规放疗．两年生存率高于常规放疗。急性消化道反应明显低于常规放疗组。更多还原

【Abstract】 Objective: To compare the difference between 3DCRT and conventiondl RT for cervical cancer. Methods: between Junnary and August 2001 30 patients with cervical cancer received 3DCRT.A1I patients received a treatment planning computer tomography scan. The targeet volume ( upper vagina. Parameterial tissues.pre-sacral region. Uterus and regional lymph nodes) and normal tissues (bladder,and rectum) were identified. Redplan 3.5 was used. Provide coverage of the target with the prescription does 45Gy. Acute gastrointestinal (GI ) and genltourinany (GU) toxic effect were compared with those in 50 patient treated with conventional RT. Results: 3DCRT provided excellent coverage of the target Providing Considerable sparing of the bladder rectum.Grade 1 GI was 83.33 %. 3DCRT had lower rate of grade 2 GI tox-icity (16.66.00% VS 26.00% P<0.01) compared with conventional RT. While grade 2 GU. Toxicety was lower in 3DCRT group (13.33%VS 16.00% ), not statistical Significcnce (P<0.01). The 2-year survival rate of 3DCRT group is significantly higher than conventional RT group (86.20%VS77.08% P<0.01). Conclusion: 3DCRT provedes excellent coverage of the towget structrue while sparing critical neighboring structures in cervical cancer, the 2-year survival rate of 3DCRT has a good result更多还原
