淋巴瘤患者外周血T/NK细胞ζ链水平的研究

【摘要】 目的:检测初治淋巴瘤患者外周血T/NK细胞ζ链水平,探讨其与淋巴瘤发生、进展及临床疗效的关系,为生物免疫治疗奠定基础。方法:用流式细胞仪检测40例淋巴瘤患者、30例健康志愿者外周血T淋巴细胞和NK细胞ζ链的平均荧光强度(MFI)。用CD3单抗、ζ链单抗和小鼠IgG γ单抗标记,流式细胞术测定T淋巴细胞和NK细胞ζ链的表达水平。结果:淋巴瘤患者组较健康对照组T/NK细胞的ζ链水平显著降低(P<0.05)。淋巴瘤患者外周血中NK细胞表达ζ链的MFI要强干T淋巴细胞。淋巴瘤患者I、Ⅱ期较Ⅲ、IV 期两组T/NK细胞的ζ链水平相对显著升高(P<0.01)。CR 组T/NK细胞的ζ链水平高于PR组(P<O.01)。淋巴瘤患者外周血中T/NK细胞ζ链水平存在显著的直线相关(P<0. 01),相关系数r=0.661。结论:淋巴瘤患者外周血中T/NK 细胞的ζ链水平降低与淋巴瘤的免疫逃逸、发生有关,且ζ链水平降低、CD4+CD25+ Treg增高与淋巴瘤细胞的侵袭能力、进展及临床疗效相关。
【Abstract】 Objective: To study the ξ chain level of T and NK cells in peripheral blood of patients with lymphoma and investigate the relationship between the ξ chain level and tumor occurrence progression and its clinical effect in lymphoma. Methods:The peripheral blood mononuclcear cells in 39 patients with lymphoma and 30 healthy blood donors were examined to the ξ chain MFI of T and NK cells by flowcytometry with fluorescent anti CD3(for T cells), anti ξ chain and anti mouse IgG γ monoclonal antibodies. Results: (1) The ξ chain expression of NK cell were stronger than T cells which suggested the higher ξ chain protein level of NK cells’ membrane. (2)T and NK cell ξ chain levels in 82.1 % (32/39) and 79.5% (31/39) of the lymphoma patients were lower than the healthy control, respectively .There was a significant difference in ξ chain expression levels of T and NK cell between the lymphoma patients and the healthy control (p <0.05). ( 3 ) There was a significant difference in ξ chain expression of T cells and NK cells in stage I, II and III, IV lymphoma ( P<0.01 ).The ξ chain expression of T cells in stage I, II and III, IV decreased to 93% and 43.1 % ;that of NK cells in stage I, II and III, IV decreased to 94.2% and 47.7% of the healthy control level, respectively. ( 4) After the regular treatment, there was a significant difference in ξ chain expression levels of T and NK cells between CR and PR lymphoma patients ( P<0.01). ( 5 ) There was a significant correlation in ξ chain expression between T and NK cells( P<0.01 ), r =0.661 . Conclusion: Reduced T and NK cells ξ chain levels of the lymphoma patients are gemerally consistent with tumor progression and correlated with clinical effect. There was a significant correlation in ξ chain expression between T and NK cells. The ξ chain level of T and NK cells is related to the mechanism of immune escape, and tumor origination, progression and its clinical effect in lymphoma.
