子宫内膜异位症潜在生物学恶性特点及发病机制探讨

【摘要】 目的:探讨子宫内膜异位症恶性潜能极其临床病理、生物学特征和生长激素、表皮生长因子受体在其发病机制中的作用。方法:应用免疫组织化学S—P法,检查人生长激素(GH)、表皮生长因子受体(EGFR)在84例子宫内膜异位症患者的在位及异位内膜组织的表达。结果:GH在在位及异位内膜组织中的阳性率分别为90.77%(59/65)和 85.71%(72/84);EGFR在在位及异位内膜组织中的阳性率分别为81.54%(53/65)和89.29%(75/84),EGFR在异位内膜组织中的阳性率高于在位内膜组织的阳性率,但无统计学意义(P>0.05)。结论:子宫内膜异位症是由多种原因引起的良性疾病,但其在生物学上也拥有肿瘤组织的一些特点,如局部浸润、远处转移、对其他组织的破坏等。G H、 EGFR子宫内膜异位症的在位及异位内膜组织中均有较高的阳性表达,表明其直接或间接地参与了子宫内膜异位症的发病过程,在其发生、发展及恶变过程中起重要促进作用。

【Abstract】 Objective: To investigate the malignant potential characteristics and pathogenesis of endometriosis in biology, the function and role of human growth hormone and epidermal growth factor receptor in the pathogenesis of malignant transformation of endometriosis were studied. Methods: Immunohistoche- mical S-P method was used to examine the expressions of human growth hormone and epidermal growth factor receptor in the eutopic and ectopic endometrium of 84 cases of endometriosis. Results: the percentage of positive cases of human growth hormone in eutopic and ectopic endometrium was 90.77% (59/65), 85.71 % (72/84) respectively; the percentage of positive cases of epidermal growth factor receptor in eutopic and ectopic endometrium was 81.54%(53/65), 89.29%(75/ 84) respectively. The positive rate of ectopic endometrium of epidermal growth factor receptor is higher than that in the eutopic endometrium but no statistical difference (P> 0.05). Conclusion: Endometriosis is caused by multiple factors, which possesses some biologic features of neoplasm such as invasion and metastasis. Both human growth hormone and epidermal growth factor receptor are highly expressed in eutopic and ectopic endometrium, which lead directly or indirectly to the pathogenesis of endometriosis and promote the initiation and malignant transformation of endometriosis.
