【摘要】 食管癌和贲门癌在我国是多发病,许多患者就诊时已属晚期,部分患者属放疗、化疗难以控制或术后吻合口复发进食梗阻,临床医师缺少良策,食管癌铂金电极针,为此开辟了一条新的治疗途径。我院从1994年5月至1998年7月, 用铂金电极针治疗晚期食管癌18例,贲门癌术后吻合口复发 6例,均取得满意疗效。其中2例食管胸中段癌晚期不能手术的患者,经电化疗后行胃和食管行左颈部吻合术,获得成功。 (1)术前禁食水,肌注杜冷丁、硫酸阿托品,鼻咽部地卡因喷雾麻醉。(2)电极针表面涂石蜡油后经鼻腔缓缓插入食管腔内,待有阻力感觉时即停止。尽管电化学能杀灭癌细胞,但它仍是一种局部治疗的手段,仍应坚持肿瘤的综合治疗原则,以提高治愈率、减少复发、延长生存期。更多还原

【Abstract】 Esophageal carcinoma and cardiac carcinoma are frequently encountered disease .Many patients belong to advanced stage when visited .Radiotherapy and chemotherapy are unavailable, some patients with stoma recur are obstructed, platinum electrode needle for esophageal carcinoma can be taken as a new treatment. From 1994.5 to 1998.7 we treated 18 patients with advanced esophageal carcinomas and 6 patients with cardiac stoma recur by platinum electrode needle, all had good effect. Therapeutic method (1) abrosia before operation, inject Sauteralgyl, astropine sulfate, spray amethacaine on pharynx nasalis. (2) Apply paraffin oil on electrode needle and then insert esophagus slowly, stop when feeling resistance. Therapeutic result (1) relieve obstruction : 3 patients relieved completely, can eat liquid diet or semiliquid, 1 case was invalid. 16 cases can intake semiliquid after treatment, who can intake liquid food . 4 cases can intake common food after treatment, (2) X-ray result: 21 cases were rechecked, there were 4 cases which lumens expended 0.5 cm, 4 cases expended 0.4 cm, 13cases expended 0.2 cm, 1 advanced-stage patient died for live and brain metastases , 2 patients discharge voluntary. (3) pathological change : 2 cases were resected, the cross-section of specimen was dust, in microscope cancer cell appeared hydropic degeneration, lysis and necrosis. Though Electrochemotherapy can kill cancer cells, it is a method of regional treatment, we still should insist on the principle of combined therapy , so as to improve cure rate , reduce relapse , and prolong life span.更多还原
