【摘要】 目的:分析与年轻乳腺癌发生及预后有关的临床、病理及生物学因素,筛选出预示转归不良的相关因子。方法回顾性分析≤35岁原发性乳腺癌99例,均随访5年,随访率100％。收集临床资料,复习病理切片,并对其中63例蜡块完整者采用免疫组化方法检测ER、PR、AR、C—erbB2、 Ki67、P53及BRCA1蛋白表达。结果:5年生存率72．7％ (72／99)。BRCA1蛋白表达未发现与年龄、乳腺癌家族史相关(P>0．05)。单因素分析显示淋巴管侵犯、软组织受累、EIC、淋巴结受累、AR和C—erbB2表达情况对预后有影响,多因素COX回归分析表明淋巴结受累、AR阴性及C—erbB2过度表达是独立预后因素。结论:淋巴结状况及C—erbB2表达情况是年轻乳腺癌的强预后因素,AR可作为一种辅助预后因子,人为干预手段不能从根本上影响转归。因资料有限年轻乳腺癌的发生因素仍不明确。更多还原

【Abstract】 Objective: To study the relationship of the susceptibility and prognosis and the clinicopathoiogicai features and biological specificity for young patients with breast cancer. Methods: The data on 99 patients with primary breast cancer (≤ 35years old) were analyzed retrospectively. The patients were followed up for 5 years with a rate of 100%. The histological specimens were reviewed. The expression of ER、 PR、 AR、 C-erbB2、 Ki67、 p53 and BRCA1 were examined by immunohis-tochemistry in 63 carcinomas. Results: The 5-year survival was 72.7% (72/99). No relationship was found be- tween BRCA1 expression and age and family history of breast cancer (P >0.05 ). In univariate, the survival was related to lymphatic vessel invasion, fat involvement, node-positive, EIC, AR and C-erbB2 expression. In Cox multi-variate model, only node-positive, AR negativity and C-erbB2 overexpression were independent prognostic factors. Conclusion: Our data demonstrate that the lymph node status and C-erbB2 expression are the strong prognostic factors in young patients with breast cancer. AR may be an adjuvant prognostic factor. The therapeutic measurement do not benefit the survival radically. This finding was limited and the susceptibility for young patients remains unclear.更多还原
