天津市区20年前列腺癌发病与死亡趋势分析

【摘要】 目的:了解天津市前列腺癌发病与死亡变化情况, 为前列腺癌的防治工作提供依据。方法: 将天津市肿瘤登记报告系统搜集的1982—2001年天津市区男性居民前列腺癌发病与死亡资料录入Excel2003数据库,进行数据整理分析。发病率和死亡率的标化采用世界标准人口,计算标化率比的95%CI来检验两个标化率的差异。趋势分析采用线性回归方程拟合。结果:1.前列腺癌年平均发病率和死亡率及年龄分布:1982~2001年天津市中心六区男性居民前列腺癌年平均发病率为2.18/10万,死亡率为1.22/10万,世界人口标化率分别为1.73/10万和1.02/10万。天津市中心六区男性居民前列腺癌发病在男性恶性肿瘤发病中的排位自 1982年起的第22位,已上升至2001年的第17位。死亡则从1982年的第18位上升至第15位。前列腺癌发病占同期恶性肿瘤发病的1.01%,死亡占0.76%。2.发病率和死亡率变化情况:20年中前列腺癌发病率呈持续上升趋势, 从 1982年的1.08/10万上升至2001年的2.38/10万,平均年增长速度为7.2%,死亡率呈平稳微升趋势,从1982年的 0.76/10万上升至2001年的1.27/10万,年平均增长速度 2.6%。在1982-1991年与1992—2001年两个时间段内,前列腺癌发病率的年均变化速度分别为7.1%和7.4%,但在 2000—2001年间这一上升变化有所加速。在这两个阶段内死亡率的年均变化速度分别为4.0%和2.90,表明上升的速度有所减缓。3.发病率和死亡率的年龄变化:年龄别发病率和死亡率均从60岁起明显上升,至80岁组。达到高峰,分别为59.41/10万和36.97/10万。前后十年比较:60岁组与80岁组的发病率分别上升了54.0%和70.4%,而死亡率在80岁组上升了55.8%,在60岁组则轻微下降了3.5%。结论:天津市居民前列腺癌的发病率和死亡率呈缓慢增加趋势且发病率增加速度大于死亡率,应加强本地区发病影响因素的研究,加强人群预防措施,尤以老年人口为重点保护人群,实施有效的早期诊断和治疗,以遏止前列腺癌流行的进一步上升。

【Abstract】 Objective: The purpose of this study was to investigate recent trends in prostate cancer incidence and mortality, with reference to changes in all men, resident in Tianjin China. Methods: We used data from prostate cancer cases and deaths reported to the Tianjin Cancer Registry during 1982-2001 to examine trends in incidence and mortality among all men. Information regarding prostate cancer mortality was obtained from the center for disease control (CDC). Incidence and mortality rates were estimated for each year from 1982 up to 2001. Results: Recent trends in incidence and mortality of prostate cancer is observed for a great number of cancers in Tianjin. The average incidence and mortality of prostate cancer among men are 2.18 per 100,000 and 1.22 per 100, 000 in the period 1982-2001 in Tianjin. The world population average standardized rates are 1.73 per 100,000 and 1.02 per 100,000 in this period. There was a steady increase in the age-adjusted incidence of prostate cancer from 1982. The age-adjusted incidence increased by 7.2% per year throughout the study period, from 1.08 per 100, 000 in 1982 to 2.38 per 100,000 in 2001. The age-adjusted mortality of prostate cancer had remained constant, with a slight rise. The age-adjusted mortality increased by 2.6% per year from 0.76 per100,000 in 1982 to 1.27 per 100,000 in 2001. This increase is less than incidence rate. Tianjin experienced 7.1% and 7.4% growth in incidence from 1982-1991 and 1992-2001, respectively. Also, experienced 4.0% and 2.9% growth in mortality from 1982-1991 and 1992-2001, respectively. This increase in incidence rates accelerated in the 2000-2001. There was an obviously increase in incidence and mortality of prostate cancer among men aged over 60 years age groups, and came to a head in 80 years age group. Between 1982-1991 and 1992-2001 the age-specific incidence of prostate cancer increased by 54.0% and 70.4% in the 2 age groups, respectively. Between 1982-1991 and 1992-2001 prostate cancer age-specific mortality increased by 55.8% in 80 years age group, but with a slight descend by 3.5% in 60 years age group. Conclusion: Monitoring cancer incidence and mortality time trends is essential for cancer research and healthcare planning. Our study aimed at estimating Tianjin cancer incidence and mortality trends over the longest period available. Recent incidence and mortality patterns observed in Tianjin China suggest Prostate cancer had a pattern of a steady increasing incidence and had a remained constant, with a slight rise mortality.
