【摘要】 目的:调查鼻咽癌高发区鼻咽癌发生的危险因素, 为鼻咽癌防治提供科学依据。方法:采用EB病毒IgA／VCA 抗体阴性和EB病毒IgA／VCA抗体阳性的非肿瘤人群分别作为对照进行配对研究,问卷调查2001之后经病理组织学确诊的新发鼻咽癌病例1009例,对可疑致癌因素进行多因素非条件Logistic回归分析,探讨多因素与鼻咽癌发病的关系。结果:EB病毒抗体阴性人群:鼻咽癌家族史、家用柴火燃料、吸烟量大是鼻咽癌的危险因素,OR值依次为5867．01,4．09, 1．71。EB病毒抗体阳性人群:鼻咽癌家族史、食用咸鱼或干鱼、接触有机溶剂、家用柴火燃料、吸烟量大是鼻咽癌的危险因素,OR值依次为1536．78,257．65,4．13,2．02,1．47。结论:鼻咽癌的发生具有较为显著的遗传倾向,家用柴火燃料及吸烟是鼻咽癌发生的危险因素,经常接触有机溶剂、食用咸鱼或干鱼等环境致癌因素之下更容易被EB病毒感染。更多还原

【Abstract】 Objective: To explore the risk factors of nasopharyngeal carcinoma, two case-control study was conducted to investigation in nasopharyngeal carcinoma high incident region. Methods: Two 1:1 matched hospital-based case control study with 1009 new cases of NPC with pathologically diagnosed after January 2001 and the cases of controls were come from the same area with EBV IgA/VCA antibody positive and negative. Non-conditional multifactor logistic regression model was performed to determine independent NPC related risk factors. Results: In EBV IgA/VCA antibody negative case-control, Multi-factor conditional logistic regression model analysis revealed that NPC family history, using firewood as fuels, increased cigarettes smoking, could increase the risk of NPC. The OR value for each risk factor was 5867.01,4.09 and 1.71 respectively. In EBV IgA/VCA antibody positive case-control that NPC family history, eating salt-presented fish frequently, occupational exposure to organic solvents, using firewood as fuels, increased cigarettes smoking, could increase the risk of NPC. The OR value for each risk factor was 1536.78, 257.65,4.13, 2.02 and 1.47 respectively. Conclusion: This study prompts that nasopharyngeal carcinoma had family heredity tendency, the factors of using firewood as fuels and increased cigarettes smoking are related closely with nasopharyngeal carcinoma. The person who exposure to organic solvents and eating salt-presented fish frequently may most easily been infected by EBV.更多还原
