【摘要】 目的通过6例报告及文献复习,探讨直肠类癌特点及合理的治疗方案。方法我院收治直肠类癌6例,记载了临床表现、病理特点及治疗方法并全部进行了随访。结果直径≤1.5cm4 例,行电灼切除术或经肛局切术,无局部复发及转移;直径≥2cm有2例,行麦式术(APR),术后病理证实第一、二站淋巴结有转移,其中一例于术后3个月盆腔复发。随访年限0.5～13年。结论肿瘤大小对判定其恶性程度是很重要的尺子,肿瘤＜1cm,无溃疡,无临床症状的,很少发生转移, 可用电灼切除;1～1.9cm肿瘤转移率10％～15％,可行局部切除术;肿瘤＞2cm、中央有溃疡、有临床症状或指诊固定则具有侵袭性和转移率高(60％～80％)的特点,宜行扩大根治术。更多还原

【Abstract】 PURPOSE Through reported 6 cases and reviewed literature we want to explore pathological and clinical eharacteristics of rectal carcinoid and, to make reasonable treatment. METHODS. Our hospital have treated 6 cases of rectal carcinoid tumors, the following were noted: symptoms and clinical findins, tumor size, color and location within the rectum. All treatments and results of treatment, including recurrent disease and survival, were recorded. Follow—up visits were noted. RESULTS. Four tumors were treated by local biopsy forceps excision with or without fulguration and transanal excision, respectively. These tumors measured 0.5 to 1.5cm. There were no recurrences. Abdominoperineal resection (APR)was made on two patients, tumors measured 2 and 3cm. Lymph node metastases were presented. Local recurrence was observed in one patient three months after operation. Patients with follow—up rang 0.5～13 years. CONCLUSIONS Size is most often used guide and reliably predictive for tumors, but it is not absolute. Diameter of tumor is smaller than 1cm, no ulceration, no symptoms, little metastases in these patients, biopsy forceps excision with to without fulguration can be used; Tumor between 1 and 1.9cm carries 10～15 percent risk of metastasizing, local excision or major resection can be used; If tumor is larger than 2cm, ulcerated, fixed, symptoms presented, all are ominous features, radical surgery should be made.更多还原
