葛根素注射液溶血不良反应机理研究

【摘要】 目的:研究葛根素注射液引致急性血管内溶血不良反应的机理。方法:在现有的溶血实验的基础上建立偶发性溶血实验评价方法;应用偶发性溶血实验、体外溶血试验、免疫性溶血试验等评价方法,探讨葛根素注射液引起溶血的机理。结果:葛根素注射液偶发性溶血实验可在现有的溶血实验的基础上,通过加大给药量和观察时间,采用24只Beagle犬进行评价;体外溶血试验结果显示溶血率和葛根素浓度存在线性关系;葛根素大剂量静脉给药可引起豚鼠红细胞膜皱缩甚至破碎;但长期大剂量静脉给药,豚鼠未发生过敏反应和免疫性溶血,体内也未检测到葛根素抗体。结论:本实验建立的偶发性溶血实验评价方法,可以用于葛根素注射液引致急性血管内溶血不良反应的评价。葛根素注射液引致溶血的因素是大剂量静脉给药影响红细胞细胞膜的稳定性所致,而非免疫反应所致。
【Abstract】 Objective: To study the mechanism of AIVH induced by Puerarin Injection. Method: Adventitious hemolysis experiment, in vitro hemolysis experiment and autoimmune hemolysis experiment are used to investigate the cause of hemolysis induced by Puerarin Injection. Result: The Beagle dog’s red cells appeared haemotolysis when the puerarin was in a high concentration. The in vitro hemolysis experiment showed that puerarin concentration and hemolysis rate had a good linear relationship statistically. Puerarin administrated intravenously at the dosage of 50mg/kg caused a crenation in each guinea pig, and cell fragments appeared in blood of tow of the six guinea pigs. No allergic response or autoimmune-hemolysis occurred after a long term’ intravenous administration of puerarin with a high dosage. No puerarin-antibody had been found in blood serum too. Conclusion: Puerarin Injection can affect the stability of the red cell membrane when the drug concentration is too high in plasma, and cause hemolysis which is not the result of immune response.
