四种冲洗剂对根管内粪肠球菌清除效果的体外比较研究

【摘要】 目的:利用粪肠球菌感染模型,比较常用根管冲洗剂对根管内粪肠球菌感染的清除效果。方法:50颗人单根管离体前牙, 蒸馏水冲洗下,K3旋转镍钛锉根管预备至.06/30#。根管内接种粪肠球菌,建立体外感染模型,然后随机分为5组,每组10个根管,分别用4种化学冲洗剂(3%过氧化氢、2.5%次氯酸钠、2%氯亚明、2%氯已啶)和0.9%生理盐水(对照组)根管冲洗。冲洗前、后根管内纸尖取样,计数细菌量。随后,每组挑选2个样本观察牙本质小管内残留细菌,另8个样本观察72小时后残余细菌的复苏。结果:化学冲洗剂使根管内细菌量减少102-106倍。实验组与生理盐水组(P<0.05)、2.5%次氯酸钠及2%氯已啶与3%过氧化氢(P<0.05)组有显著性差异。牙本质小管内残留细菌,72小时后大部分根管内细菌重新长入。结论:3%过氧化氢杀菌作用较弱,2%氯己啶、2%氯亚明杀菌效果与2.5%次氯酸钠相似,4种冲洗剂均不能使根管达到完全无菌状态。
【Abstract】 PURPOSE: To compare the antimicrobial efficacy of four endodontic irrigants using an in vitro model infected by Enterococcus faecalis. METHODS: Fifty extracted human single-rooted anterior teeth were collected, and the root canals were infected by Enterococcus faecalis in vitro. Thereafter, the specimens were divided into five groups randomly, and were irrigated with 3% hydrogen peroxide(H2O2), 2.5% sodium hypochloride(NaOCl),2% chloramine-T(CR), 2% chlorhexidine(CHX) and 0.9% sodium chloride, respectively. The concentration of E. faecalis in each group of canals was calculated before and after irrigation. 2 specimens were randomly selected from each group of canals after the irrigation to observe the residual bacteria within the dentinal tubules, and the other 8 specimens were examined for survival of residual bacteria by means of incubation. RESULTS: All chemical irrigants were significantly more effective than saline solution in reducing the number of E. faecalis within the root canal (P<0.05); 2.5% NaOCl and 2% CHX were statistically more effective than 3% H2O2(P<0.05). Residual bacteria could be found in the dentinal tubules. Bacteria propagated 72h after in the canals. CONCLUSION: 2% chloramine-T and 2% chlorhexidine had almost the equivalent antimicrobial effect as 2.5% NaOCl, no irrigant could kill the intracanal E. faecalis thoroughly.
