离体上颌第一前磨牙解剖因素对根管治疗术难度的影响

【摘要】 目的:探讨上颌第一前磨牙的根管解剖形态对根管治疗术难度的影响。方法:选择100个解剖标志完好的上颌第一前磨牙,利用游标卡尺测量牙全长。然后利用RVG系统拍摄颊舌向的影像,观察根管类型和形态、髓腔和根管钙化度,并测量根管近远中向弯曲度。最后采用根管治疗术难度系数评估标准评估其对根管治疗难度的影响。结果:根管数目和根管弯曲与牙全长、髓腔和根管钙化度有显著性差异(P<0.05),上颌第一前磨牙的根管解剖形态对根管治疗术难度的主要影响因素为:根管数量和根管弯曲度。结论:诊治上颌第一前磨牙时应尤其要仔细探查根管数量和根管弯曲;准确的根管治疗术难度评估有助于根管治疗的成功进行。
【Abstract】 PURPOSE: To investigate the effect of root canal anatomy in maxillary first premolars on root canal treatment difficulty. METHODS: One hundred undamaged permanent maxillary first premolars were used. The span was measured by a sliding caliper. Then standardized periapical radiovisiography (RVG)images were taken from buecolingual direction. The root canal types and morphology, the calcification of pulp chamber and root canal system were observed, and the mesiodistal curvatures of canals were measured according to the images. The effects of root canal anatomy on root canal treatment difficulty were evaluated according to assessment standards of risk factors. RESULTS: The major risk factors on root canal treatment were root canal number and root canal curvature in maxillary first premolar. CONCLUSION: The clinicians should explore root canal number and root canal curvature carefully when diagnose and treat maxillary first premolars. Precise assessment of risk factors will conduce to successful root canal treatment.
