多层螺旋CT在复杂根管治疗中的应用研究

【摘要】 目的:探讨16排螺旋CT及其三维成像技术在复杂根管治疗中的应用价值。方法:采集来我院就诊的复杂的根管治疗病例,进行常规X线平片检查后,采用16排螺旋CT断层扫描检查,应用表面遮盖法(shadedsurface display,SSD),多平面重建 (mutiplanar reconstruction,MPR)技术进行影像三维重建。结果:和常规X线平片相比,CT断层扫描图像可准确判断根管的融合和分支情况;应用SSD技术去除软组织影像,可以在任何方向进行影像三维重建,清晰显示患牙的牙体组织和周围牙槽骨影像,能分辨出复杂瘘管、窦道的通路,并能准确测量根尖周病变的范围。结论:应用螺旋CT及其三维成像技术,可对复杂的矸髓系统病变进行准确诊断,有效提高临床医生对复杂髓腔系统病变的诊断和治疗。
【Abstract】 OBJECTIVE: To evaluate the application of helical CT and reconstruction methods in treatment of complex root canal. METHODS: All cases for root canal therapy were collected in our hospital,and received 16-slice helical CT scan after getting routine check of X ray.The data were input 3D workstation and processed with SSD and MPR imaging processing software to obtain the reconstruction images. RESULTS: Comparing with X ray check, the images of root canal interlacing and branching could be more clearly showed in scanning images with helical CT check. The 3-D dental images could be reconstituted at any direction with MPR method after shading the images of soft tissue with SSD method. The reconstruted images could show accurately the location and condition of tooth and alveolar bone, the way of fistulous tract, and the extention of apical periodontal diseases. CONCLUSION: The tooth pulp diseases could be accurately diagnosed by applying helical CT and its 3D reconstrution methods, which could improve the diagnosis and treatment of complex root canal.
