超声评价左室心肌应变率及其影响因素的临床研究

【摘要】 目的观察正常成年人左室心肌应变率(strain rate,SR)及其影响因素,为临床正确评价心肌局部收缩和舒张功能奠定基础。方法应用心肌SR成像技术,观察了55例健康志愿者(男28例、女27例,平均年龄44.5±15.6岁)不同左室壁心肌收缩期、舒张早期和心房收缩期的应变率峰值(PSSR、PESR、PASR),并分析年龄、性别、心率和体表面积对其的影响。结果①左室壁不同部位之间SR参数不存在显著性差异,提示心肌SR的部位均一性;②年龄对SR参数影响显著,与PSSR和PESR负相关(r分别为0.461和0.619)而与PASR正相关(r=0.726);性别、心率和体表面积与心肌SR相关不显著。③与其他低龄组比较,≥60岁组的PESR显著降低而PASR显著升高;PSSR各年龄间无显著性差异。④PSSR、PESR和PASR的正常值在<60岁年龄组分别为-1.38±0.18(s-1)、2.20±0.48(s-1)和1.16±0.28(s-1);在≥60岁年龄组为-1.22±0.11(s-1)、1.21±0.24(s-1)和1.81±0.15(s-1)。结论左室心肌应变率不存在部位差异,舒张期应变率参数受年龄影响显著。

【Abstract】 Objective To determine the normal values of myocardial strain rate (SR) and their related factors. Methods Fifty-five healthy volunteers were enrolled in the study (male 28 and female 27 with a mean age of 44. 5±15. 6 years). Using SR imaging, the peak SR at systole, early diastole and atrial contraction (PSSR, PESR and PASR for short respectively) were obtained and the influence of age, gender, heart rate and body surface area were analyzed. Results (1)SR parameters at different walls of left ventricle showed no significant difference, indicating a homogeneous pattern in all walls of left ventricle. (2)Age showed significant correlation with SR parameters while heart rate, gender and body surface area indicated no relation with them. Age was correlated negatively with PSSR, PESR and positively with PASR. (3)In subjects≥60 years old, PESR decreased while PASR increased significantly compared with those <60years old. No significant difference was found in PSSR among different age groups. (4)The normal values of PSSR, PESR and PASR were -1.38±0. 18(s-1), 2. 20±0. 48 (s-1) and 1. 16±0. 28 (s-1) respectively in subjects <60 years old, and were - 1. 22± 0. 11(s-1), 1.21±0.24 (s-1) and 1.81±0.15 (s-1) respectively in those≥60 years old. Conclusion Myocardial strain rate of left ventricle shows a homogeneous pattern and the age has significant influence on diastolic strain rate parameters.
