儿童医院产超广谱β-内酰胺酶菌感染现状及耐药性研究

【摘要】 为了监测儿童医院产超广谱β-内酰胺酶(ESBLs)菌感染和耐药现状,以指导临床医生合理用药,采用双纸片协同法初筛、确证法检测产ESBLs菌,并对筛选确证为产ESBLs的菌株进行药敏实验。结果表明:各种临床标本中检出大肠埃希菌和肺炎克雷伯菌1466株,其中798株为产ESBLs菌,检出率为54.4%;产ESBLs发生率最高为新生儿科(68.5%)和呼吸内科(34.2%);标本来源主要为呼吸道痰液标本(73.2%)。产ESBLs菌株对17种抗菌药物的耐药性显著高于非产ESBLs菌株,而且产ESBLs菌表现出严重耐药和多重耐药特点。因此,得出结论:儿童医院产ESBLs的发生率相当高,儿科医生应重视感染性标本的早期培养,并根据药敏结果合理使用抗生素,以便及时有效地控制ESBLs菌株在医院感染与流行。

【Abstract】 In order to evaluate infection and drug resistance status of extended-spectrum β-lactamases(ESBLs) producing strains in children hospital, ESBLs producing strains were screened by double-disk synergy test and detected by confirmatory test. Drug resistance test were then performed by K-B method. RESULTS 1466 strains were isolated from the clinical specimens, including 848 strains Escherichia coli and 618 strains Klebsiella pneumoniae. 54.4% of these strains, that is 798 strains, were confirmed to produce ESBLs. The rate of producing ESBLs strrain in newborn department was 68.5%, and that in respiratory diseases department was 34.2%. These ESBLs producing strains were mostly isolated from sputum(73.2%). The resistant rate of ESBLs producing strain to 17 antibiotics were much higher than that of non ESBLs producing strain, and most of the ESBLs producing strain showed multi-drug resistance. The prevalence of ESBLs in children hospital infection was high, so the clinical microbiology laboratory should pay heed to its detection for rational use of antibiotics and effective control of resistance.
