运动对慢性冠状动脉狭窄后侧枝循环生成的影响

【摘要】 目的 冠心病的康复治疗中运动疗法一直是主导模式,但是其中心效应尚未得到公认.本研究的目的是在建立慢性冠状动脉狭窄模型的基础上进行运动训练研究其对冠状动脉侧枝循环生成的影响.方法 健康三月龄猪18只(体重22.5±2kg),手术放置 Occluder于冠状动脉左回旋支(LCX)主干上以形成慢性冠状动脉狭窄模型.术后恢复4周,然后通过平板运动分别训练4周、6周和8周后采用微粒体技术观察缺血心肌的局部血流改变,并通过病理学检查观察形态学改变.结果(1)大体标本证实术后存活超过4周的13只猪全部稳定形成LCX完全闭塞;(2)微粒体检测证实运动组LCX血管支配区心肌局部血流量较对照组明显增加;(3)病理学检查证实运动组LCX血管支配区毛细血管密度增加;(4)训练结束后运动组猪的平板运动能力较对照组明显增加.结论 运动可以促进慢性冠状动脉狭窄模型的冠状动脉侧枝生成增加.

【Abstract】 Objective Exercise therapy is the major approach in rehabilitation of coronary artery disease. But its central effect is not generally acknowledged. This research was to study the effect of exercise on collateral on chronic coronary stenosis model. Method Eightenn pigs(3 months old, 22. 5 + 2kg weight), were resulted in coronary stenosis models by Ameroid Occluder constricted proximal left circumflex coronary artery(LCX). After 4 weeks recovery, they were grouped in 4 teams and 3 of 4 teams were exercised on treadmill respectively for 4 weeks, 6 weeks and 8 weeks. When exercise training was performed, the microspheres were injected into left atrium to measure the local blood flow(LBF) of myocardium and then myocardial tissue was collected to observe morphologic change. Result (l)Complete obstruction in LCX was shown in all pigs that survived surgery for more than 4 weeks. (2)Microspheres measuring showed that LBF strengthened obviously in the area that LCX perfused. (3)Morphologic examining showed that density of blood capillary increased in the area that LCX perfused. (4)Exercise abilities were enhanced in all exercise groups.Conclusion Exercise can promote collateral growth in chronic coronary stenosis model.
