髋关节置换术后早期康复
Rehabilitation of post hip replacement

【摘要】 目的 探讨髋关节置换术后的康复治疗.方法 从1999年7月至2001年3月共68例,男23例,女45例,年龄27—89岁,平均66岁.病种:髋部骨折34例,股骨头缺血坏死13例,骨性关节炎10例,类风湿性关节炎2例,术后松动9例.骨水泥全髋关节置换术19例,非骨水泥全髋关节置换术30例,股骨头置换术19例(骨水泥固定17例),术后2周内早期康复包括:踝泵,股四头肌、腘绳肌、外展外旋肌群肌力训练,ROM训练,体位转换,坐立及步态训练,ADL,出院前教育.结果 术后2周出院,肌力Ⅳ—V级,髋关节伸屈0°—90°,骨水泥固定组负重行走(非骨水泥固定组术后6周负重),基本生活自理.结论 髋关节置换术后早期康复对患者肌力、ROM、ADL的恢复具有显著疗效,可以预防并发症,促进患髋关节功能的早日恢复.
【Abstract】 Objectives To explore the rehabilitation following hip replacement. Method Forty -two kneew in Sixty - eight cases(23 males, 45 females) were treated at this group, aging 27 - 89(average 66) years old. It included hip fracture 34 cases, avascular necrosis of the femoral head 13 cases, os-teoarthritis 10 cases, rheumatoid arthritis 2 cases and loosen implant 9 cases. The surgeries included non-cemented total hip replacement 30 cases, cemented total hip replacement 19 cases and semi hip replacement 19 cases. The early stage training focused on ankle pumps, strength of the quadriceps femoris muscle, hamstring muscle, abductor and gluteal muscles, ROM(extension and flexion), transfers, sitting, standing, walking, ADL and education before going home. Results At 2 Weeks postoperation patients got Grade IV-V in muscle strength of the involved leg, 0 - 90 AROM, weight bearing walking(cemented group)and basic independent ADL Conclusion Early stage rehabilitation training postoperation promotes the recovery of the muscle strength of the involved leg, ROM and ADL, which prevent the complications postsurgery and make the patients get good hip function early.
