用WADE法造成脊髓损伤中枢性疼痛的动物模型

【摘要】 目的 中枢性疼痛是脊髓损伤(Spinal Cord Injury,SCI)的难治性合并症,使患者痛不欲生.但由于迄今为止尚未找到恰当的动物模型进行研究,使其机制未明,临床限于对症处理,是其难于治疗的主要原因.为了研究其发生机制以寻找治疗的根本方法,我们采用WADE法建立了SCI中枢性疼痛的动物模型.方法 选用健康成年Wistar雌性大鼠46只,随机分为两组(X1,X2组).用Alien’s的WADE法造成脊髓L2-3阶段的300gcm(X1)和200gcm(X2)的损伤,损伤前后分别记录体感诱发电位.于术前和术后12小时、16小时、24小时、每天清晨用Von Frey Hair等测定大鼠的对轻触和机械性轻压的痛阈及前足抬足率的阈值的变化,并观察大鼠自发的其他痛行为学的变化.两组均对于伤后痛行为进行了平均4个月的观察.结果 两组均出现了典型的急慢性痛超敏现象(手术前后痛阈值的比较,具有显著或非常显著性差异:(P<0.05-0.001)和自发痛现象,其中X1组与X2组比较,发生率更接近人类损伤.结论 用WADE法造成大鼠300gcm脊髓损伤是重复性好且具有与人类损伤和疼痛最为接近的典型的SCI的中枢性疼痛的动物模型.

【Abstract】 Objectives Central Pain(CP)is a complication in Spinal Cord Injury(SCI), which is hard to be treated. It makes patients sufferings even want to suicide. But there is not suitable animal model, so it’s mechanism hasn’t made clear yet. The clinic management to it limited in expectant treating, which is the main cause of difficulty to treat. In order to research it’s mechanism and to look for excellent treating method, we set up an animal model of CP in SCI by WADE method. Method 46 female ratwere divided into 2 groups: XI and X2. Make lumber segment of spinal cord(L2 - 3) injury by WADE method using 300gcm(Xl) and 200gcm (X20and record SEP before and after injury. Assess pain threshold to mild tactile, mechanical pressure sense & rate of withdrawing forefoot using Von Frey Hair 12 hour, 16 hour, 24 hour after operation and every morning for about 4 months. We also observe the behavior change of spontaneous pain & so on. Results Two groups exhibited typical acute and chronic allody-nia phenomina( Comparison of pain threshold before & after operation was found to have significance difference; P< 0 . 05 - 0.001). Conclusion 300 gem SCI to rat by WADE method has excellent reappearance and is the closest to SCI & CP in SCI in human, and has typical CP in SCI. Therefore, it is an excellent animal model of CP in SCI.
