小组工作介入宫颈癌患者生活质量提升的研究

【摘要】 2020年,世界卫生组织发布了《加速消除宫颈癌全球战略》,全球各个国家和政府也积极响应世界卫生组织的号召,支持该战略。宫颈癌作为影响全球女性健康的第四大癌症,严重影响着广大妇女及家庭的生活质量。随着生物-心理-社会医学模式以及全人健康理念的发展,各个学科对于宫颈癌患者生活质量的研究也提上了日程。本研究采用参与式观察法、半结构访谈法以及问卷法,依据身心社灵全人健康模式,以H医院宫颈癌患者为服务对象,运用小组社会工作实务方法,在深度剖析了宫颈癌患者的问题及需求的基础上,有针对性地开展小组活动,以希望能提升患者的生活质量。本研究发现,宫颈癌患者面临的问题主要有疾病与治疗带来的生理疼痛、面对疾病时产生的心理压力、所需的社会支持网络不完善以及对未来生活的希望水平较低,导致问题的原因包括对疾病存在认知偏差、缺少发泄情绪途径等,正是这些问题致使宫颈癌患者生活质量不高,进而影响后续的治疗与康复。因此,本研究从生理、心理、社会和灵性四个角度出发,以患者需求为导向,设计并开展六节小组活动,以提升宫颈癌患者的生活质量。本研究证明,小组工作对于提升宫颈癌患者的生活质量是有效的,组员的生理疼痛得到缓解、心理压力得到降低、社会支持得到增强、灵性健康得到提升。社会工作者采用非医疗的治疗方法介入,有效地满足了患者生理、心理、社会以及灵性层面的需求。最后,笔者就此次小组工作的介入进行了反思,在医院内开展小组活动,需要以患者需求为导向,并要求医务社会工作者理清自身角色、掌握一定的专业技巧以应对突发情况,增强自身协调组织以及抗压能力,这会取得更好的效果。

【Abstract】 In 2020,the World Health Organization released the Global Strategy to Accelerate the Elimination of Cervical Cancer,and countries and governments around the world actively responded to the call of the World Health Organization and supported the strategy.Cervical cancer,as the fourth largest cancer affecting women’s health worldwide,seriously affects the quality of life of women and families.With the development of the biopsychosocial medical model and the concept of holistic health,research on the quality of life of cervical cancer patients in various disciplines has also been put on the agenda.This study used participatory observation,semi-structured interview,and questionnaire methods,based on the holistic health model of physical,mental,social,and spiritual well-being,with H Hospital’s cervical cancer patients as service objects.Using group social work practice methods,based on a deep analysis of the problems and needs of cervical cancer patients,targeted group activities were carried out to improve the quality of life of patients.This study found that the main problems faced by cervical cancer patients include physiological pain caused by the disease and treatment,psychological pressure when facing the disease,incomplete social support networks required,and low levels of hope for future life.The reasons for these problems include cognitive bias towards the disease,lack of emotional pathways to vent,etc.It is these problems that lead to low quality of life for cervical cancer patients,which in turn affects subsequent treatment and rehabilitation.Therefore,this study starts from four perspectives:physiological,psychological,social,and spiritual.Guided by patient needs,six group activities are designed and carried out to improve the quality of life of cervical cancer patients.This study demonstrates that group work is effective in improving the quality of life of cervical cancer patients,as the physiological pain of group members is relieved,psychological pressure is reduced,social support is enhanced,and spiritual health is improved.Social workers intervene using non medical treatment methods,effectively meeting the physiological,psychological,social,and spiritual needs of patients.Finally,the author reflected on the intervention of this group work.To carry out group activities in hospitals,it is necessary to be patient oriented and require medical and social workers to clarify their roles,master certain professional skills to cope with emergencies,enhance their coordination,organization,and stress resistance abilities,which will achieve better results.
