【摘要】 目的:探讨阴茎癌合理而有效的诊疗方式。方法:总结分析2012年1月至2018年12月期间于青海大学附属医院泌尿外科和青海省人民医院泌尿外科住院并行手术治疗的80例阴茎癌的患者临床资料特点。结果:鳞状细胞癌64例,乳头状瘤样增生恶变8例,疣状癌6例,恶性黑色素瘤2例。行阴茎局部病变切除术者26例,行阴茎部分切除术者40例,行阴茎全切加会阴部尿道造口术者14例。对其中50例肿大的淋巴结进行了活检,其中32例活检阳性。28例术中同期行双侧腹股沟淋巴结清扫术,4例同期行盆腔淋巴结清扫术。80例定期随访1～3年,平均随访19个月,1、2、3年总体生存率分别为95%(76/80)、90%(72/80)、80%(64/80)。结论:外科手术是治疗阴茎癌合理而有效的处理方式。淋巴结的转移状态是关系患者预后的重要因素。一旦并发腹股沟区、髂淋巴结转移,应积极考虑行淋巴结清扫术。更多还原

【Abstract】 Objective: Exploring the reasonable and effective diagnosis and treatment of penile carcinoma.Methods:The clinical data of 80 patients with penile carcinoma who were hospitalized in the Urology Surgery Department of青海Hospital and the Urology Surgery Department of青海Hospital from Jan.2012 to Dec.2018 were summarized and analyzed.Results: There were 64 cases of squamous cell carcinoma,8 cases of papilloma hyperplasia and malignant transformation,6 cases of verrucous carcinoma and 2 case of malignant melanoma.Partial penile lesion resection was performed in 26 cases,partial penile resection in 40 cases,and total penile resection plus perineal urethrostomy in 8 cases.50 cases of the enlarged lymph nodes were biopsied,32 cases of which were positive.Bilateral inguinal lymph node dissection was performed in 28 cases and 4cases received pelvic lymphadenectomy.The 80 cases were followed up regularly for1 to 3 years,with an average follow-up of 19 months.Moreover,the overall survival rates during 1,2 and 3 years were 95%(76/80),90%(72/80)and 80%(64/80),respectively.Conclusion: At present,surgical operation is a reasonable and effective treatment for penile carcinoma.The status of lymph node metastasis is an important factor related to the prognosis of patients.Lymph node dissection should be actively performed in patients complicated with inguinal and iliac lymph node metastasis.更多还原
