微小染色体维持蛋白7在垂体腺瘤中的表达与机制研究

【摘要】 目的:本研究通过免疫组化检测垂体腺瘤组织中MCM7蛋白表达水平,分析MCM7蛋白在垂体腺瘤发生发展中的作用,以及MCM7与垂体腺瘤各个临床和病理因素之间的关系。探讨其作用机制,进而为未来垂体腺瘤治疗的新方法探寻理论依据。方法:垂体腺瘤标本来自河北医大第二医院266例鞍区占位手术取材的标本中随机抽样80例病理标本,按肿瘤分泌功能分组,其中GHPA20例,PRLPA20例,NFPA40例。同时所有80例样本又可根据Knosp标准分类侵袭性和非侵袭性组。符合I、II级的样本纳入为非侵袭性腺瘤组,符合III级和IV级的样本纳入为侵袭性垂体腺瘤。其中IPA腺瘤29例,NIPA共51例。阴性对照组正常垂体组织1例来自河北医大第二医院神经外科解剖实验室。通过免疫组化技术来检测垂体腺瘤组织的中MCM7蛋白的表达水平和蛋白定位。结果:1 MCM7免疫组化结果示MCM7蛋白在正常垂体组织细胞染色阴性;2 MCM7表达在80例垂体腺瘤免疫组化平均评分为3.75±2.74,其中有功能性垂体腺瘤评分(6.11±2.89),其中GH腺瘤(6.88±2.31)和PRL腺瘤(5.54±1.47)。无功能垂体腺瘤评分(1.78±1.66)依据侵袭性分组评分:非侵袭性垂体腺瘤评分(6.24±2.89)高于侵袭性垂体腺瘤(1.76+1.64)。统计推断结果,功能性垂体腺瘤评分高于无功能垂体腺瘤,两者差异有统计学意义(P<0.05)。NIPA组评分高于IPA组评分,两者之间差异具有统计学意义(P<0.05)。3 MCM7在各亚型垂体腺瘤均有表达,染色集中在细胞核和细胞浆,沿核膜分布。结论:1 MCM7蛋白在各类型PA组织中均表达。2 MCM7蛋白在有分泌功能的垂体腺瘤垂体腺瘤表达水平高,无功能表达水平低。在NIPA中表达水平高,在IPA表达水平低。3垂体腺瘤中MCM7不再是胞浆中均匀分布,而是沿核膜分布,MCM7很可能是通过影响P53的功能进而促进了PA细胞的增殖。
【Abstract】 Objective:In this study we used immunochemistry to detect MCM7 protein expression in pituitary adenoma.Furthermore,MCM7 and their clinic pathological significance of pituitary adenoma have been explored.Methods: Eighty cases of surgically resented pituitary adenoma specimens were collected from HeBei province the second hospital from2015.11 to 2016.11.A11 specimens had been confirmed by clinical and pathological diagnosis.The patients were classified histological as 20 cases of PRL adenoma,20 cases of GH adenoma,40 cases of NF adenoma.According to the Knops classification,29 cases belong to invasive pituitary adenoma,51 cases belong to noninvasive pituitary adenoma.Using immunochemistry,we measured MCM7 in tumor tissue from patients with pituitary adenoma.Results:1 In immunochemistry,MCM7 protein can not be detected in normol pituitary tissue.2 In immunochemistry,MCM7 protein was detected in cytoplasm and nucleus.The score of MCM7 was the highest in NIPA(6.11±2.89),as compared with other tumors.IPA exhibited the lowest expression levels,thus the comparison with other three tumor subtypes demonstrates a great statistical significance(P<0.05).The expression of MCM7 had no statistical differences in different sex,tumor Size and invasiveness(P>0.05).3 In immunochemistry,MCM7 protein was detected in cytoplasm and around the nucleus.Conclusions:1 MCM7 protein were all detected in pituitary adenoma.2 The expression level of MCM7 are higher in NIPA and function pituitary adenoma.3 In conclusion,in this work we supposes that Mcm7 is a novel P53 interacting protein.Mcm7 binds to P53 and inhibits P53 mediated transcription.
