【摘要】 研究背景与目的:结肠癌一种是我国常见的恶性肿瘤,其发病率逐年上升。近年来,我国对于结肠癌的治疗水平有明显提高,主要表现在手术切除率、根治切除率、术后生存率的提高,以及术后并发症的减少。腹腔镜辅助下手术治疗早期结肠癌的可行性、安全性、优越性已在大量文献中得到证实。但对于治疗已有淋巴结转移,肿瘤体积较大的Ⅲ期结肠恶性肿瘤,其术中的安全性、肿瘤根治的彻底性及术后的恢复都有争议,且腹腔镜手术较开腹手术的成本高,性价比也存在着争议,因此腹腔镜辅助下手术仍不是治疗Ⅲ期结肠癌的“金标准”。本研究通过比较腹腔镜辅助下手术与开腹手术治疗Ⅲ期结肠癌术前、术中、术后的数据资料,以达到探讨腹腔镜辅助下手术治疗Ⅲ期结肠癌的安全、可行性的目的;方法:本研究收集了我院自2012年1月到2013年12月111例收治的已行手术根治的Ⅲ期结肠癌患者的临床资料,将其分为开腹组和腹腔镜组(其中开腹组52例、腔镜组59例),两组数据的术前资料无显著差别(年龄、性别、ASA分级、术前合并症P<0.05),通过术中、术后相关数据(术中出血量、术后排气时间、术后住院时间、术后切口感染率、费用、短期复发转移率)的比较以达到研究目的;结论:腹腔镜治疗Ⅲ期结肠癌是安全可行的。更多还原

【Abstract】 Background and destination:colon cancer is very common in our country.Its incidence rate is rising with the time going on.In resent years,more and more patients with colon cancer have been diagnosed and treated.A lot of studies have proved the feasibility,safety and superiority of laparoscopic surgery to early colon cancer.But to the third phase of colon cancer,the feasibility,safety and superiority of laparoscopic surgery are suspected by many people as the Lymph node have transfered and the size of cancer is rather big,also the cost is more than open surgery,all of those lead to the result of laparoscopic surgery not becoming standard method therapy to the third phase of colon cancer. This study is ready to lead us to know whether it is feasible and safe or not through comparing two group of datum of open surgery and laparoscopic surgery.Methods:This study analyzed a database containing the information about patients who have accepted surgery therapy for the third phase of colon cancer from January 2012 to December 2013 in our hospital.Thisdatabase was consisted of 52 cases of open surgery and 59 cases of laparoscopic surgery.The clinical datum of the two groups before surgery have no significant differences,we compare the clinical datum before and after surgery of the two groups to support our result of the study.Conclusion:laparoscopic surgery is feasible and safe for the third phase of the colon cancer.更多还原
