精浆中酞酸酯类代谢产物与精子质量及血清生殖激素水平间的相关性研究

【摘要】 【目的】以精浆中酞酸酯类(phthalic acid esters,PAEs)代谢产物浓度作为生殖靶器官暴露标志,在中国人群中探讨环境PAEs暴露对精子质量及血清生殖激素水平的影响。【方法】以来武汉市同济医院生殖中心行精液质量检查的男性为研究对象,收集其精液(n=687)和血液样本(n=342),并测定精子质量参数和血清生殖激素水平。采用高效液相色谱串联质谱法测定精浆中8种PAEs代谢产物浓度。运用多元线性回归和logistic回归模型分析精浆PAEs代谢产物浓度与精子质量参数和血清生殖激素水平之间的相关性。【结果】精浆PAEs代谢产物浓度增加与精液体积减少[邻苯二甲酸单乙酯(mono-n-butyl phthalate,MBP)、邻苯二甲酸单(2-乙基己基)酯(mono-(2-ethylhexyl)phthalate,MEHP)、邻苯二甲酸(2-乙基-5氧基己基)酯(mono-(2-ethyl-5-oxohexyl)phthalate,MEOHP)和邻苯二甲酸(2-乙基-5羟基己基)酯(mono-(2-ethyl-5-hydroxyhexyl)phthalate,MEHHP)]、精子曲线和直线运动速率降低[MEHP、邻苯二甲酸单卞酯(monobenzyl phthalate,MBzP)、邻苯二甲酸双(2-乙基己基)酯(di-(2-ethylhexyl)phthalate,DEHP)代谢产物中MEHP所占百分比(%MEHP)]以及精子头尾部畸形率增加(MBzP)之间存在剂量反应关系(趋势P值均<0.05)。精浆PAEs代谢产物浓度与血清生殖激素水平之间的相关性均无统计学意义。【结论】本次研究结果提示环境水平的PAEs暴露可能会损害男性精液质量。

【Abstract】 Goal: To examine the associations between phthalate exposure encountered in the environment and semen quality and reproductive hormones among C hinese men, using phthalate metabolite levels in seminal plasma as biomarkers to estimate exposure status of target reproductive organs.Methods: Semen(n=687) and blood samples(n=342) were collected from men who presented to the Reproductive Center of Tongji Hospital to seek semen examination. Semen quality parameters and serum levels of reproductive hormone were assessed. Seminal plasma levels of 8 phthalate metabolites were determined using high-performance liquid chromatography and tandem mass spectrometry. Multivariate linear and logistic regression models were used to assess the associations between seminal plasma metabolites and semen quality parameters and serum reproductive hormone levels.Results: Increasing seminal plasma metabolites were associated with decreasing semen volume [mono-n-butyl phthalate(MBP), mono-(2-ethylhexyl) phthalate(MEHP), mono(2- ethyl-5-oxohexyl) phthalate(MEOHP), mono(2-ethyl-5-hydroxyhexyl) phthalate(MEHHP)], sperm curvilinear velocity [MEHP, monobenzyl phthalate(MBz P), the percentage of di-(2-ethylhexyl)-phthalate metabolites excreted as MEHP(%MEHP)] and straight- line velocity(MEHP, MBz P, %MEHP), and also associated with increasing percentages of abnormal sperm heads and tails(MBz P)(all Ptrend <0.05). No significant associations were observed between seminal plasma metabolites and serum reproductive hormone levels.Conclusion: Our results suggest that exposure to phthalates at environmental levelsmay impair human semen quality.
