【摘要】 目的基于中医“五神”学说，结合“形神合一”观及现代身心医学的概念，研制《中医“五神”老年性痴呆评定表》，并试以“志意”为切入点，探讨老年性痴呆的病机层次及证候要素，为老年性痴呆的临床辨识辨证提供一种简洁有效的中医评价体系，并为老年性痴呆的治疗提供一种新的思路和方向。方法借助于电子检索《中华医典》、网络检索CNKI医学文献及手工检索《四库全书》等途径,进行大规模的文献调研,形成备选词条，经专家Delphi法规范论证形成条目池，再经专家组论证及小样本备试构建出由80个条目组成的《中医“五神”老年性痴呆评定表》的初始评定表，通过对临床预调查数据的统计分析结果，对评定表条目进行筛选修证，然后再通过进一步的大样本临床流调对修证评定表进行再调整及科学性考核，最后制定出信效度和灵敏度良好的正式《中医“五神”老年性痴评定表》；最后结合正式评定表的数据分析结果，以中医“五神”学说之“志意”为切入点，揭示老年性痴呆的病机特色与证候要素。结果研制出了信效度和灵敏度均较为理想的《中医“五神”老年性痴呆评定表》，该评定表能够客观、全面地涵盖老年性痴呆患者“五神”病变的临床表现，并结合统计分析结果，以“志意”为切入点，对所抽取的老年性痴呆的病机层次及证候要素进行了全面阐述，为老年性痴呆提供了一种新的临床辨治思路；本评定表语言简洁明了，操作简单，经济实用。更多还原

【Abstract】 Objective Develop “Alzheimer’s disease evaluation table to the‘five directional gods’of TCM as the theoretical basis” based on the theory of “five directional gods” ofTCM,combined with the thought of “harmonization between soma and spirit” and theconcept of psychosomatic medicine, and take “zhiyi” as the breakthrough point to discussthe Pathogenesis levels and syndrome factor of the pathogenesis of based on the theory of“five dir-ectional gods” of TCM,to provide a concise and effective evaluation system oftraditional Chinese medicine for Alzheimer’s Ddisease clinical identifi-cation of dialecticaland for the treatment of Alzheimer’s Disease offers a new thought and direction.MethodsMake a largescale literature survey research and get optional entries By the way ofretrieving “the medicine” and the Chinese National Knowledge Infrastructure and theimperial collection of four divisions,and then form the entries pool through the expertsDelphi method standard argument,and then construct an initial assessment of the“Alzheimer’s Disease evaluation table to the‘five directional gods’ of TCM as thetheoretical basis” which consisting of80entries through the demonstration of the expertgroup and small sample prepared,and then assess the table entries on screen according tothe investigation of clinical data of statistical analysis of the results,and then according tofurther large sample clinical epidemiological data analysis results of the revised evaluationtable to adjust the structure of the evaluation table and scientific evaluation,and at lastwork out a formal “Alzheimer’s Disease evaluation table to the‘five directional gods’ ofTCM as the theoretical basis” of good reliability and validity and sensitivity,and thenreveal the Pathogenesis levels and syndrome factor of the pathogenesis of Alzheimer’sdisease by choosing the “zhiyi”of the “five directional gods”of TCM as the bre-akthrough point and based on the formal evaluation table data analysis re-sults.Results Successfullywork out a “Alzheimer’s disease evaluation table to the‘five directional gods’ of TCM asthe theoretical basis” with good reliability and validity and sensitivity,this evaluation tablecan objectively and comprehensively covers patients with dementia “five directionalgods”the clinical manifestations of Alzheimer’s disease,and make a full explanation of theextraction of the Pathogenesis levels and syndrome factor of the pathogenesis ofAlzheimer’s disease combining with the results of the survey and in “zhiyi” as thebreakthrough point,and provide a new clinical differentiation idea for senile dementia,theoperation is simple,economic and practical.更多还原
