【摘要】 目的了解国家基本药物制度在淮北地区试点基层医疗机构的推行现状,对进一步完善国家基本药物制度提供建议。方法采用处方资料的回顾性研究方法,通过对抗高血压药物利用研究,对安徽省淮北地区国家基本药物制度试点的全部19所乡镇卫生院的基本药物制度推行情况进行调查。所有资料采用SPSS11.5进行统计分析,技数资料统计描述主要采用构成比指标,计量资料的描述以x±s表示,计数资料组间比较采用x2检验。并对调研结果进行综合评价,综合探讨基本药物使用的实效与合理用药的关系。结果(1)抗高血压基本药物处方率达到93.84%,其中以卡托普利的使用最为广泛,使用率为86.4%。药品使用的构成比及基本药物处方率在制度实施前后未见明显差异(x~2=3.152,P>0.05);(2)2009年10～12月与2010年1～3月抗高血压药的药品金额下降较为显著(U=-15.689,P<0.05)。结论我省基层医疗机构基本药物实行药品零差率销售,能规范使用,制度实施前后药品费用的下降具有统计学意义,但对基层医疗机构医师的处方行为未见明显影响,基本药物使用的品种数较单一。建议加强基本药物的宣传和培训;有针对性地制订农村各级医疗机构基本药物目录,规范用药品种,指导乡村医生规范处方;理顺农村基层医疗机构补偿机制,合理构建基本药物在农村的保障供应体系。更多还原

【Abstract】 Objective To understand the implementation status of essential medicines system in primary health care sector of rural areas, focusing on understanding the supply situation of essential drugs, and provide advice to imporve the national essential medicines system.Methods Combined with questionnaire survey and retrospective study of prescription data, set hypertension medicines use as target,using stratified cluster sampling method,investigate the implementation of the system of essential medicines in 19 township hospitals in HuaiBei in Anhui Province. All data were analyzed using SPSS11.5, measurement data indicated as mean±standard deviation (±s). Result Essential medicines prescriptions rate of hypertension in primary health care sector is 93.84%,of which the most widely used of captopril, the use rate of 86.4%. Conclusions Essential medicines of primary health care sector in Anhui Province implement zero slip pharmaceutical sales, to regulate the use, but the basic number of drug use than a single species.Suggestion Strengthen publicity and training of essential medicines;improve the supply of essential drugs in rural areas; have targeted development of rural medical institutions of essential medicines list at all levels , standard drug varieties; guide norms of rural doctors prescription; straighten out the compensation mechanism for rural primary health care institutions.更多还原
